| Objective:Epithelial ovarian cancer is most fatal gynecological disease.With the continuous progress of early diagnosis,drug research and clinical treatment,patients still face a higher risk of recurrence.This study conducted a retrospective analysis of the clinical history and follow-up data of patients with epithelial ovarian cancer,in order to explore the impact of epithelial ovarian cancer-related factors on the interval between patients after surgery.For future clinical diagnosis and treatment,improve the prognosis of postoperative follow-up and provide a reliable reference.Methods:This research collected clinical data of 120 patients from January 2010 to January2020.These epithelial ovarian cancer patients received initial ovarian cancer surgery and supplemented with chemotherapy in the Department of Gynecology,Northern Jiangsu People’s hospital.The follow-up date is until January 1,2021.The calculated disease-free interval is based on the time of the first postoperative recurrence.In addition,analyze the effects of age,preoperative menopause,clinical stage,ascites volume,tumor size,pathological type and other factors on postoperative disease-free interval.Furthermore,to study the correlation between disease-free interval and overall survival.Statistical analysis software selects SPSS24.0.Explore the related factors that affect the disease-free interval after epithelial ovarian cancer.Result:1.Univariate analysis suggests that: ascites volume,pathological type,ascites cytology,histological grade,preoperative platelet count,clinical stage,number of chemotherapy courses are related to disease-free interval,and all of these factors have significant differences(P<0.05).The patient’s age,preoperative menopause,and tumor size were not significantly correlated with disease-free interval(P > 0.05).Earlier clinical stage,higher level of histological differentiation,no significant increase in ascites volume,and standard and adequate chemotherapy after the first postoperative,no cancer cells in ascites cytology,the longer the postoperative disease-free interval of the patient.The clinical stage,histological grade and the number of chemotherapy courses were significantly correlated with postoperative disease-free interval through multivariate analysis(P<0.05).2.The cut off of CA125 group boundary value with 994.35 U/ml was calculated by ROC curve and Youden index.There were 70 patients with preoperative serum CA125<994.35U/ml,28 patients died as of the follow-up date,and the median overall survival was 85 months.There were 50 patients with CA125≥994.35U/m L and 40 died at the end of follow-up.The median overall survival was significantly reduced to 40 months in the group with CA125≥994.35U/ml(P<0.05).3.The preoperative CA125 value,clinical stage,histological grade and the number of chemotherapy courses were tested separately by log-rank method,and the difference in survival rate after recurrence of epithelial ovarian cancer was analyzed.The results showed that there were significant differences(P<0.05).4.The disease-free interval of patients with epithelial ovarian cancer after surgery was significant correlation with the overall survival(rs=0.707,P < 0.05),that is the longer the postoperative disease-free interval,the longer the overall survival.Conclusion:1.Ascites volume,pathological type,ascites cytology,histological grade,preoperative platelet count,clinical stage,number of chemotherapy courses are related to the postoperative disease-free interval of patients with epithelial ovarian cancer.The independent influencing factors include three factors: clinical stage,histological grade,and the number of chemotherapy courses.2.The preoperative tumor marker serum CA125 level is highly expressed in epithelial ovarian cancer,indicating that the higher the degree of tumor malignancy,the shorter the overall survival.3.There are also correlations between clinical stage,histological grade,number of chemotherapy courses and overall survival.4.The longer the postoperative disease-free interval of patients with epithelial ovarian cancer,the longer the overall survival. |
