Objective:To detect the expression of Sema4 D in bone marrow and peripheral blood of children with acute leukemia,and to analyze the relationship between the expression of Sema4 D and clinical characteristics of children.To explore the clinical significance of Sema4 D in the occurrence,development and prognosis of children with acute leukemia.To find new targets for treatment of children with acute leukemia.Method:This is a prospective study,87 cases(ALL 66 cases,AML 16 cases,relapse 5 cases)were collected from peripheral blood or bone marrow specimens of Al patients and recurrent AL patients in the Haematological Oncology Department of Kunming Children’s Hospital,peripheral blood from 28 children with nonmalignant disease and bone marrow from 8children with thrombocytopenia disease served as controls.The expression of Sema4 D in peripheral blood and/or bone marrow was detected by western blot,q PCR and ELISA assay.Sex,age,risk grade,early remission,chromosome karyotype,fusion gene and other clinical features were collected to analyze the correlation between Sema4 D expression and clinical features.Result:(1)Compared with the controls,the expression of Sema4 D was higher in AL patients(primary group and recurrent group)(p < 0.05).(2)The expression of Sema4 D in different types of acute leukemia(ALL,AML,Relapse Group)had no significant difference(P > 0.05).(3)The expression level of Sema4 D in different types of ALL was different.The expression of Sema4 D between T-ALL and B-ALL was significantly different in peripheral blood and bone marrow(p < 0.05).(4)As the chemotherapy progressed,children with ALL showed varying degrees of remission,and the level of Sema4 D expression in bone marrow showed a decreasing trend.The expression level of Sema4 D was significantly different in different stages of chemotherapy(p < 0.05).(5)Pearson correlation analysis showed that the level of Sema4 D expression in ALL serum was positively correlated with the number of white blood cells and immature cells in peripheral blood(figure F:r=0.430,P =0.004;figure G:r=0.390,P =0.011).Conclusion:(1)Sema4D was highly expressed in AL patients,but there was no difference in Sema4 D expression among different types of acute leukemia.Sema4 D may be related to the onset and recurrence of leukemia.(2)Sema4D can be used as a reference index to distinguish ALL immunophenotype.(3)The expression of Sema4 D decreased after chemotherapy in children with ALL.Sema4 D may be a potential indicator of ALL early remission.(4)The higher the number of white blood cells and immature cells,the higher the level of Sema4 D expression.Sema4 D may be involved in the proliferation of ALL tumor cells.
