| Background and objective:Malignant melanoma(MM)is an malignant tumor originating from the spinal melanocytes.The transformation from melanocytes to MM is a complex process,which requires complex interactions among exogenous,endogenous,tumor internal environment and immune-related factors.The malignant degree of MM is very high.In the early stage,the treatment of MM is mainly chemotherapy,and the prognosis is not ideal.In recent years,the emergence of targeted therapy and immunotherapy has greatly improved the prognosis of patients with MM.However,MM is a tumor with high heterogeneity and ethnic differences.Even in the same region,the epidemiology,clinicopathological characteristics and prognosis are also greatly different with different anatomical sites.Widespread studies have been published in Western countries,and MM has been reported in many parts of Asia,including China.However,there has been no relevant report on MM in Northeast China,and the epidemiology,clinicopathological characteristics,survival and post-treatment outcomes of MM are largely unknown.Therefore,more information about the characteristics and treatment of this disease in Northeast China is needed in order to better understand this highly aggressive and ethnically specific malignancy.Methods:Collection on January 1,2013-September 1,2020 Jilin medical university first hospital,after the removal of pathologic examination and biopsy specimens of clinical and pathological diagnosis of MM patients,treatment and prognosis of data,the application of SPSS analysis,understand MM epidemiological characteristics,clinical and pathological characteristics and various types of MM difference,survival condition and the influencing factors of relapse and the influencing factors of survival,the curative effect of Advanced first-line therapy.Results:1.Two hundred and twenty nine MM patients ranged in age from 19 to 88 years old,with a median age of 57 years old and a peak age of 51 to 60 years old.There were more females than males.2.Cutaneous type and ocular type patients were younger,skin type and mucosal type patients were more female,skin type was more prone to BRAF mutation,mucosal type patients were more prone to recurrence,and acrotype bone metastasis was more common;3.Overall survival(OS)of the 229 patients with MM was 90.1%,73.1% and56.9%,respectively,after 1,3 and 5 years.The rates of DFS at 6,12,18 and 36 months were 88.9%,77.3%,67.3% and 51.2%,respectively.4.OS and DFS of ocular type were better than other types,mucosal type was the worst,and the recurrence rate of mucosal type was higher than other types,accounting for 83.6%(P=0.008).5.Age and stage at first diagnosis were independent prognostic factors for OS,while disease free survival(DFS)was the only independent prognostic factor for OS.Conclusion:1.Single center MM patients in Northeast China,the peak of onset was 51-60 years old.Patients in this age group,especially those with a high number of nevi,should improve their awareness of self-screening and seek medical treatment in time.2.The overall survival rate of single center MM patients in Northeast China was lower than the data reported abroad,but better than the domestic statistical data ten years ago.3.The prognosis of ocular type MM is better than that of other types,which is different from that reported at home and abroad.The reason may be that ocular type patients are less included.The prognosis of mucosal type is the worst,and the recurrence rate of mucosal type is higher than other types,which is similar to domestic and foreign reports.4.Age and stage at first diagnosis were independent prognostic factors for OS,while stage at first diagnosis was the only independent prognostic factor for DFS,and stage at diagnosis was an important prognostic index. |
