Objective:The purpose is to observe whether the clinical efficacy of ambroxol through ultrasound conduction transdermal administration and conventional intravenous and aerosol administration is different in the clinical efficacy of CAP in the elderly,and to explore its clinical guiding significance.Methods:1.Research objects:A total of 160 elderly patients aged ≥65 years who were admitted to the respiratory department of Jilin Provincial People’s Hospital and diagnosed with CAP from January 2017 to December 2020 were retrospectively included.2.Research method:According to the different administration methods of ambroxol,they were divided into four groups,namely the control group,the intravenous group,the superconducting group,and the atomization group.There were 40 patients in each group.In terms of treatment,each group was given anti-infection(piperacillin),maintaining the homeostasis(including water-electrolyte and acid-base balance),and given symptomatic treatments such as oxygen inhalation,antipyretic,antispasmodic and antiasthmatic treatments as needed.Among them,the control group was not treated with ambroxol,the superconducting group was treated with ambroxol transdermal administration(15mg,bid)via ultrasound conduction,the intravenous group was treated with intravenous ambroxol(30mg,bid),and the nebulization group was treated with ammonia.Bromoxol nebulized treatment(15mg,bid).Observe the changes of infection-related indicators(WBC,NE%,CRP,PCT)before treatment and 7 days after treatment,the degree of CT imaging changes,and the number of days for symptom relief(cough,sputum,wet rales in the lungs).3.Statistical method:Statistical methods: use SPSS22.0 software for data processing and statistical analysis.Results: :A total of 160 patients were enrolled in this study,with an average age of(74.33±6.27)years old.They were divided into four groups according to the different administration methods of ambroxol,namely the control group,the intravenous group,the superconducting group,and the nebulization group.There were 40 patients in each group.1.The intravenous group has the best effect on reducing white blood cells,followed by the superconducting group,and finally the nebulization group.The three groups are better than the control group;P<0.05,there is a difference between the two groups.2.In terms of the decrease in NE%,the intravenous group is compared with the superconducting group(P>0.05),that is,the efficacy of the two groups is equivalent,and both are significantly better than the atomization group(P<0.05),and the efficacy of the control group is still worse than these three groups.3.In terms of reflecting the difference before and after CRP treatment,pairwise comparisons are statistically significant(P<0.05).The intravenous group(99.95±3.54)is better than the superconducting group(90.21±2.39),followed by the nebulization group(80.01±1.79),the control group had the worst curative effect(69.81±2.75).4.Judging from the decrease of PCT index before and after treatment,the comparison between groups is statistically significant(P<0.05).The intravenous group(698.87±11.34)has the largest decrease in PCT,followed by the superconducting group(599.98±9.45).Then the nebulization group(599.98±9.45),and finally the control group(394.57±31.99)the PCT decreased the least.5.According to the lung inflammation absorption rate data of each group,the lung CT after 7 days of treatment was P<0.05 compared with that before treatment.The intravenous group had the highest absorption rate of pneumonia lesions(54.93±28.72),followed by the superconducting group(45.08±19.99),Then the nebulization group(35.05±20.72),and finally the control group(30.63±18.90).6.There are differences in the number of days of symptom relief in the four groups(P<0.05).The intravenous group(8.18±2.02)has the shortest number of days,followed by the superconducting group(8.60±2.75),and then the atomization group(9.20±2.95)The control group(9.63±2.33)is the longest.Conclusion:1.In the basic treatment of elderly CAP,the addition of ambroxol can significantly enhance the efficacy,speed up the control of infection,and shorten the course of the disease.2.The effect of transdermal administration of ambroxol by ultrasound conduction is equivalent to that of intravenous ambroxol to a certain extent,and both are better than atomized administration,and the operation is non-invasive,safe,and has higher compliance for elderly patients.Expect wider application.
