Objective:To explore the value of AFP-L3,DCP and GP73 in the diagnosis and evaluation of HCC,and to clarify the significance of combined diagnosis of tumor markers and its correlation with tumor volume and vascular invasion.Method:The clinical data of 91 patients with HCC admitted to Jilin University Hospital from January 2020 to December 2020 were analyzed retrospectively.41 patients with non hepatocellular carcinoma were included as control group.The serum levels of AFP,AFP-L3,DCP and GP73 were measured by magnetic particle chemiluminescence immunoassay,and the ratio of AFP-L3 to total AFP(L3%)was calculated.The ROC curve of single and joint detection was drawn,and the area under the curve was calculated.The clinical value of AFP-L3,DCP and GP73 was compared with that of single monitoring AFP in the diagnosis of HCC.The correlation between tumor markers and size of HCC tumor and vascular invasion was determined by linear correlation analysis and positive rate.Result:1.The levels of AFP,DCP,GP73,AFP-L3 and L3%in HCC group were higher than those in the control group,and the results of DCP and L3%were statistically different(P<0.05).2.The positive rates of AFP,DCP,AFP-L3 and L3%in HCC group were higher than those in the control group(P<0.05),and there was no significant difference in GP73 positive rate between the two groups(p=0.933>0.05).3.When the serum AFP,DCP,GP73,AFP-L3 and L3%were used to diagnose HCC alone,the sensitivity and specificity of AFP-L3 were the best,and the area under AFP curve(AUC)was the highest;the sensitivity and specificity of the combined diagnosis were not significantly different from that of the single detection(P>0.05);the sensitivity of the combined diagnosis of AFP+DCP+GP73+L3+L3%was 0.791,the specificity was0.732,AUC was 0.857.4.DCP(r=0.683,R ~2=0.467,P<0.001)was positively correlated with the size of the tumor,that is,the higher the tumor was,the higher the serum DCP level was.5.The DCP and AFP-L3 ratio in the vascular invasion group were significantly higher than those in the non-vascular invasion group(P<0.05).The positive rates of GP73 and AFP-L3 were statistically significant(P<0.05).Conclusion1.Compared with AFP,GP73 and AFP-L3,DCP and L3%(AFP-L3/AFP)have more advantages in the identification of liver cancer;2.The detection rate of HCC can be improved by the combination of AFP,DCP,GP73,AFP-L3 and L3%,which can improve the detection rate of HCC and reduce the missed diagnosis rate;3.The level of blood clearance DCP is positively correlated with the tumor size;4.The increase of serum DCP and L3%ratio was more common in patients with HCC and vascular invasion,and the increase of serum GP73 and AFP-L3 provided a direction for the invasion of blood vessels. |
