ObjectiveObjective to explore the relationship between apolipoprotein E(ApoE)gene polymorphism,low-density lipoprotein(LDL)and high-density lipoprotein(HDL)subclasses and susceptibility,differential value and risk factors of liver cancer,so as to provide basis for early diagnosis and risk assessment of liver cancer.MethodsMethods:From December 2018 to December 2020,113 patients with primary liver cancer and 70 patients with viral hepatitis in our Hospital and 80healthy people in physical examination center were selected.They were divided into liver cancer group,hepatitis group and control group.Serum samples were collected to detect ApoE gene polymorphism,LDL and HDL subclasses by gene chip and protein electrophoresis.Divide the enrolled subjects into E2,E3,and E4 groups according to ApoE alleles,and analyze the basic information.The distribution of ApoE gene and the levels of LDL and HDL subclasses were compared between liver cancer group and hepatitis group.The relationship between ApoE gene polymorphism and susceptibility of liver cancer and the differential value of LDL and HDL subclasses for liver cancer and hepatitis were analyzed The risk factors of HCC were preliminarily screened by analyzing the indexes between groups,and the relationship between multiple factors and the risk of HCC was further analyzed by logistic regression.Results1.The genotype of?3/?3 was the most common in liver cancer group,hepatitis group and control group,and the genotype of?2/?2 was the least common.The genotype of?2/?2 was not detected in control group,and the frequency of?2 allele in liver cancer group was higher than that in hepatitis group and control group,the difference was statistically significant(P<0.05).2.Among the enrolled patients,there was no significant difference in gender,age and smoking factors among different ApoE gene phenotypes(P>0.05),which was comparable;obesity and drinking were related to liver disease,and the difference was statistically significant(P<0.05).3.The levels of large and loose low density lipoprotein(IbLDL)and high density lipoprotein 2(HDL2)in liver cancer group were higher than those in hepatitis group,while the levels of small and dense low density lipoprotein(sd LDL)and high density lipoprotein 3(HDL3)in liver cancer group were lower than those in hepatitis group(P<0.05).The AUC of IbLDL,sd LDL,HDL2and HDL3were 0.701,0.548,0.650 and 0.741,respectively,indicating that HDL3is the most valuable in the differential diagnosis of liver cancer and hepatitis4.IbLDL≥45.75,HDL2≥25.72 and?2 allele were the risk factors of HCC(P<0.05).ConclusionsThe frequency of?2 allele in hepatoma group was 10.62%,which was significantly higher than that of the healthy control group and hepatitis group,which indicated that the people with the?2 allele were susceptible to liver cancer.Obesity and drinking can lead to high liver disease rates,but not independent risk factors for liver cancer.The alleles of IbLDL,HDL2and carrying?2 were positively correlated with the risk of liver cancer,and HDL3level was more valuable for the differentiation of liver cancer and hepatitis compared with IbLDL,sd LDL and HDL2.
