Objective: To evaluate the short-term(6-8 weeks)effect of selective lateral episiotomy on pelvic floor structure of parturient women by three-dimensional pelvic floor ultrasound.Methods: A total of 54 parturients who underwent ultrasound examination and selective perineotomy at 6-8 weeks after delivery in our hospital were retrospectively collected as the perineotomy group,according to maternal age,body weight,newborn weight and so on.A total of 54 parturients were selected as control group(non-perineotomy group).According to the way of 1:1 pairing,54 parturients who gave birth via vagina and did not undergo perineotomy were selected as the control group(non-perineal lateral episiotomy group).The distances from the bladder neck and the posterior wall of the bladder to the posterior inferior edge of the pubic symphysis were measured by two-dimensional combined three-dimensional pelvic floor ultrasound mode in the resting state and the maximum Valsalva movement,and the movement of the bladder neck was calculated,the posterior angle of the bladder was measured and the funnel formation of the internal urethra was observed,the distance from the anterior or posterior lip of the lower cervix to the pubic symphysis in the maximum Valsalva movement was measured,and the protrusion height of the rectal ampulla relative to the internal sphincter of the anus was measured.The parameters of levator ani muscle hiatus were measured and the differences between the two groups were compared.Results:Under the maximum Valsalva movement,there was no significant difference in the posterior horn of bladder between the experimental group and the control group,and there was no significant difference in the movement of bladder neck and the formation rate of urethral funnel between the two groups(P > 0.05).Under the maximum Valsalva movement,the posterior wall of bladder,cervix and rectum in the experimental group were lower than those in the control group,and the difference was statistically significant(P < 0.05).Under resting state and maximum Valsalva movement,the LHLR,LHAP,LHC and LHA of the experimental group were lower than those of the control group,and there was significant difference between the two groups(P < 0.05).Conclusions: Selective perineotomy has a certain protective effect on the structure of pelvic floor in the near future,so it is necessary to perform perineotomy when there are clear indications..
