| Purpose: To explore the diagnostic efficacy of AI and doctors of different seniority in the diagnosis of coronary artery stenosis ≥50%,and to clarify its diagnostic value for coronary heart disease.Methods: A total of 130 suspected CHD patients receiving CCTA in our hospital were collected continuously.Using ICA as the gold standard control,the diagnostic value of AI and doctors with different seniority in CHD was quantitatively evaluated.Results: Based on patient level,AI,low seniority and high seniority doctors were 91.3%,93.3% and 97.1%,respectively.The specificity was 80.8%,80.8%,84.6%,and AUC was 0.861,0.870,0.909.Compared with ICA,AI,doctors with low seniority showed general diagnostic consistency(Kappa value was 0.68,0.72,respectively),while doctors with high seniority showed better diagnostic consistency(Kappa value was 0.83).Based on the level of coronary branches,the sensitivity of AI,low seniority and high seniority doctors were 83.8%,82.7% and 91.6%,respectively.The specificity was 93.9%,91.5%,93.3%,and AUC was 0.888,0.871,0.925.All three were consistent with ICA in the diagnosis of CHD(Kappa values were 0.79,0.75,0.84,respectively).Based on the level of coronary artery segments,the sensitivity of AI,low seniority and high seniority doctors was 77.2%,79.7% and 89.3%,respectively.The specificity was 96.3%,94.7%,96.6%,and AUC was 0.888,0.871,0.925.Compared with ICA,AI and doctors with high seniority had better diagnostic consistency(Kappa value was 0.75,0.84,respectively),while doctors with low seniority had general diagnostic consistency(Kappa value was0.73).In terms of the time of image reconstruction and analysis,AI took the shortest time[(3.03±0.23)min],while the time of doctors with low seniority took the longest [(13.18±1.55)min].Conclusion: The sensitivity of AI in the diagnosis of CHD is similar to that of doctors with low seniority,and the time of image reconstruction and analysis is less than that of doctors.Therefore,AI can be used as one of the effective methods for screening CHD,but the diagnosis of occlusion of blood vessels should be cautious. |
