ObjectivePancreatic cancer is a common malignant tumor of the digestive system in the clinic.It has the characteristics of high incidence and poor prognosis.The survival rate of patients within five years is less than 5%.Pancreatic cancer lacks specific symptoms in the early stages,and the rate of missed diagnosis is up to 71%.Many patients have advanced to advanced stage when diagnosed,and delaying the optimal treatment time is an important cause of quality of life decline and death.The study found that early and accurate diagnosis of pancreatic cancer and effective treatment measures can improve the survival rate to more than 46% within five years.Therefore,accurate early diagnosis of pancreatic cancer and clear staging are the keys to improving quality of life and prognosis.In recent years,tumor markers have played an important role in the early diagnosis of digestive tract tumors,especially vascular endothelial growth factor(VEGF),transforming growth factor-β1(TGF-β1),carbohydrate antigen 19-9(CA19-9),The carbohydrate antigen 242(CA242)and the secreted protein Dickkopf-1(DKK1)have received extensive attention in the differential diagnosis and staging prediction of pancreatic cancer.There are limitations to the detection of a single indicator,and the combined detection can greatly improve the diagnostic accuracy.This study will analyze the diagnostic value of combined examination of VEGF,TGF-β1,CA19-9,CA242,and DKK1 in order to provide a reference for the early diagnosis and treatment of clinical pancreatic cancer.Methods49 patients with pancreatic cancer treated in our hospital from February 2016 to February 2019 were selected for study and recorded as the lesion group,and 57 healthy adults were selected as the control group.All patients were eligible for inclusion and Exclusion criteria.All selected subjects participated in the research on a voluntary basis and signed an agreement,which was reviewed and approved by the hospital medical ethics committee.Serum specimens were collected and processed,serum levels were measured,serum VEGF,TGF-β1,CA19-9,CA242,and DKK1 levels were compared between the two groups,and VEGF and TGF were analyzed by receiver operating characteristic curve(ROC)and Logistic regression-β1,CA19-9,CA242 and DKK1 single and combined detection of diagnostic value,analysis of serum VEGF,TGF-β1,CA19-9,CA242,DKK1 levels and pathological characteristics of patients with pancreatic cancer.SPSS 21.0 software was used to process the research data,and the test standard was P <0.05.ResultsThe CA19-9 level in the lesion group was(334.41 ± 24.58)U / mL higher than the control group(10.26 ± 1.32)U / mL;the VEGF level(378.49 ± 138.42)ng / L was higher than the control group(178.65 ± 35.47)ng / L;TGF-β1 level(32.14 ± 2.64)μg was higher than the control group(11.65 ± 2.44)μg;CA242 level(221.64 ± 26.95)U /mL was higher than the control group(10.69 ± 3.96)U / mL;DKK1 level(5.12 ± 2.16)Ng / mL was higher than the control group(2.09 ± 0.68)ng / mL,and the difference between the two groups was statistically significant(P <0.05).VRGF level has nothing to do with gender,age,tumor site,tissue differentiation,tumor size(P> 0.05),and TNM stage(P <0.05);TGF-β1 level and gender,age,tumor site,tissue differentiation,tumor The size was not related(P> 0.05)and related to the TNM stage(P <0.05);the CA19-9 level was not related to gender,age,and tumor size(P> 0.05),and related to the TNM stage,tissue differentiation,and tumor location(P < 0.05);CA242 level has nothing to do with gender,age,tumor site,tissue differentiation degree,tumor size(P>0.05),and TNM stage(P <0.05);DKK1 level and gender,age,tumor site,tissue differentiation,There was no correlation between tumor size and TNM stage(P> 0.05).The ROCs of TGF-β1,VEGF,DKK1,CA242,CA19-9,and pre-1 were 0.909、0.896、0.785、0.847、0.895 and 0.951,respectively.The sensitivity of TGF-β1,VEGF,DKK1,CA242,CA19-9 and the combined detection are 85.70%、81.63%、79.59%、77.55%、75.50%,95.92%;specificity is94.74%、96.49%、89.47%、91.23%、96.49%、94.74%;accuracy is 90.57%、89.62%、84.91%、84.91%、86.79%、95.28%.ConclusionsThe expression levels of VEGF,TGF-β1,CA19-9,CA242 and DKK1 in patients with pancreatic cancer are significantly higher than healthy adults,which is helpful for the differential diagnosis of early disease.At the same time,the expression levels of VEGF,TGF-β1,CA19-9,CA242 and DKK1 are closely related to the pathological characteristics of pancreatic cancer,which can be used as an effective basis for judging the disease stage.The individual detection of each indicator has important value for the diagnosis of pancreatic cancer,but the combined detection of the five indicators can further improve the sensitivity and accuracy,and has higher application value.
