| Objective To investigate the effects of isolate hypothyroidism in the middle and third trimester of pregnancy on glucolipid metabolism and fetal growth,and provide advice on clinical treatment.Methods Collect thyroid function test results of antenatal care and childbirth pregnant women in our hospital obstetric from 2018 to June 2020.According to established specific reference range during pregnancy by our hospital,select pregnant women with low thyroid hormone levels as experimental group.According to detect gestational week,the experimental group is divided into mid pregnancy and late pregnancy group,select healthy pregnant women with normal thyroid function as the control group,Comparing groups of pregnant women metabolic indicators(blood glucose,cholesterol,triglyceride,HDL and LDL),pregnant women body mass index(progestational BMI,prenatal BMI and increased BMI during pregnancy),the pregnancy complications(the incidence of gestational diabetes,gestational hypertension,preeclampsia),the pregnancy outcomes(the incidence of premature birth,low birth weight,macrosomia and cesarean section)and fetal growth index(head circumference,abdomen circumference,birth weight and birth BMI).Results 1.Blood glucose and triglycerides in the mid and late pregnancy groups were higher than those in the control group;2.The pre-pregnancy BMI,prenatal BMI and increased pregnancy BMI in the mid and late pregnancy groups were significantly higher than those in the control group;3.The incidence of gestational diabetes in the mid pregnancy group was significantly higher than that in the control group;There was no difference in gestational hypertension and preeclampsia among all groups.4.The incidence of preterm birth in the late pregnancy group was higher than that in the control group;The incidence of cesarean section in the mid and late pregnancy groups was higher than that in the control group,but there was no difference in the incidence of low birth weight and macrosomia among all groups.5.The birth weight of neonates in the group(persistent hypothyroxidemia)was higher than that in the control group.Conclusion 1.Isolate hypothyroidism during pregnancy increases the risk of glucolipid metabolism,mainly manifesting in blood glucose and triglyceride;2.Isolate hypothyroidism during pregnancy increases the risk of diabetes,premature delivery and cesarean section,whether it increases the risk of high birth weight or even macrosomia needs further study. |
