| PurposeApillary thyroid carcinoma accounts for more than 90%of malignant thyroid tumors.Although the prognosis is good, local lymph node metastasis is easy to occur in the early stage. Some studies have shown that age, gender, tumor size, and distant metastasis are important prognostic factors for papillary thyroid carcinoma.Tumor budding is closely related to the prognosis of malignant tumors, but the budding of papillary thyroid carcinoma has not been studied yet. This study investigated the correlation between sprouting papillary thyroid carcinoma and the correlation of CK19 protein expression, showing that the sprouting level of thyroid cancer and the clinicopathological characteristics of thyroid papillary carcinoma are closely related to the degree of tumor malignancy.,May affect the prognosis of patients.MethodsA total of 319 patients underwent radical thyroidectomy in Chifeng Cancer Hospital from 2012 to 2020.Research requirements:None of the patients received radiotherapy or chemotherapy before surgery, clinical pathological data, and medical records were complete.Patients were divided into two groups according to the degree of tumor budding.HE staining and CK 19 staining were analyzed separately.Observe the budding phenomenon of the tumor under a light microscope, and regularly follow up the survival of patients after surgery.SPSS 21.0 statistical software was used for analysis, and the correlation between tumor buds of papillary thyroid carcinoma and clinicopathological factors and prognosis was observed and recorded.ResultsAmong the 319 patients with papillary thyroid carcinoma studied, analytical data showed that 132 patients had no tumor buds and 187 had tumor buds.According to the Ueno standard method, the experiment was divided into 2 groups,126 cases in the low bud group(1-9/HPF)and 61 cases in the high bud group(≥10/HPF).In this study, the tumor diameter was divided according to the maximum diameter. There were 249 patients with a diameter of<1 cm,and 70 patients with a diameter of≥1 cm.The pathological results showed that there were 121 postoperative pathological patients with capsule invasion under the microscope,87 postoperative patients with lymph node metastasis under the microscope,48 patients with both capsule invasion and lymph node metastasis;132 patients had bilateral lesions 187 people had unilateral lesions.Among these 187,37 had multiple lesions and 150 had single lesions.According to this study, postoperative pathological tumor sprouting is closely related to its envelope invasion and lymph node metastasis(P<0.05),and other factors such as gender, age, tumor size, and unilateral/bilateral tumor occurrence There is no obvious relationship on the lesion(P>0.05).Conclusions1.In this experiment,it is the first time to reveal whether there are sprouting,low-grade tumor sprouting and high-grade tumor sprouting in papillary thyroid carcinoma tissue, which has not been reported before;2.Tumor budding of papillary thyroid carcinoma has nothing to do with gender, age, tumor size(1cm),unilateral and bilateral tumors;3.The sprouting of papillary thyroid carcinoma is related to capsule invasion and lymph node metastasis, and these factors are one of the signs of malignant degree of malignant tumor. And the degree of sprouting can better predict the clinical lymph node metastasis, which has a clear relationship with the prognosis. |
