ObjectiveTo study the predictive value of systemic immune inflammation index(SⅡ)in the overall survival rate of patients with pancreatic ductal adenocarcinoma after Whipple surgery.MethodsThe clinical data of patients with pancreatic ductal adenocarcinoma who underwent Whipple surgery in the First Affiliated Hospital of Zhengzhou University from January 1,2010 to December 31,2017 were retrospectively analyzed,and the SⅡ value was calculated.The best cut-off value of SⅡ was 900,and all patients were divided into low SⅡ group(SⅡ<900)and high SⅡ group(SⅡ>900)with 900 as the boundary value.The Kaplan-Meier method was used to draw the survival curve and log-rank test was used.The test analyzes the overall survival time of the two groups of patients,using the Cox risk regression model to perform univariate analysis of various clinicopathological parameters,and perform multivariate analysis of statistically significant indicators.ResultsA total of 135 patients were enrolled,including 78 males and 57 females,aged 28.0~76.0(56.6±8.8)years old.The 135 patients with pancreatic ductal adenocarcinoma were divided into low SII group(SⅡ≤900)and high SⅡ group(SⅡ>900),with 92 cases and 43 cases respectively.The Kaplan-Meier survival curve was used to analyze the overall survival rate of patients with pancreatic ductal adenocarcinoma in the low SⅡ group and the high SⅡ group.The median survival time of the two groups was 32.7 months(95%CI 28.4~37.0)and 24.4 months(95%CI 21.4~27.4),the survival rate of patients with pancreatic ductal adenocarcinoma in the low SII group was significantly higher than that in the high SⅡgroup,and the overall survival rate between the two groups was statistically significant(P<0.05).In univariate survival analysis,it was found that preoperative high SⅡ in peripheral blood(HR=2.047 95%CI:1.354~3.096)was significantly correlated with the shortened overall survival(P<0.05).COX multivariate analysis verified that preoperative high SⅡ in peripheral blood(HR=1.672 95%CI:1.094~2.555)is still an independent prognostic factor affecting overall survival(P<0.05).ConclusionSⅡ is an independent prognostic factor available before surgery for patients with pancreatic ductal adenocarcinoma.It has certain guiding significance in predicting the overall survival of patients with pancreatic ductal adenocarcinoma after Whipple surgery.
