| ObjetiveTo evaluate the efficacy and safety of homoharringtonine(HHT)in the treatment of acute myeloid leukemia(AML)by meta-analysis.MethodsWe searched Pub Med,the Cochrane Library,CNKI,Wanfang medical network and other databases to collect prospective / retrospective control studies of homoharringtonine containing regimen in the treatment of acute myeloid leukemia.The search time was from the establishment of the database to October 2020.A total of 37 studies involving 2846 patients with acute myeloid leukemia were included.The experimental group was treated with homoharringtonine,and the others were included in the control group.The overall response rate(ORR),complete remission(CR),cumulative incidence rate(CIR)and early death rate(EDR)were analyzed by Stata 12.0 software.ResultsMeta analysis showed : compared with the control group,the overall response rate(OR = 1.78,95% CI: 1.48-2.14,P = 0.000)and complete remission rate(OR =1.25,95% CI: 1.06-1.47,P = 0.009)of acute myeloid leukemia patients treated with homoharringtonine were improved.In addition,the incidence of infection was lower than that of non homoharringtonine group(OR = 0.77,95% CI: 0.63-0.95,P = 0.014).Compared with the control group,the cumulative incidence rate(or = 0.93,95% CI:0.58-1.49,P = 0.770),early death rate(OR = 1.50,95% CI: 0.81-2.77,P = 0.198)and the incidence of grade III-IV myelosuppression(OR = 0.88,95% CI: 0.60-1.29,P =0.514)were not significantly different between the experimental group and the control group..Conclusion1.Compared with the regimen without homoharringtonine,the overall response rate and complete remission rate of the regimen with homoharringtonine were higher,and the incidence of infection was lower,especially for relapsed and refractory patients;2.Compared with chemotherapy without homoharringtonine,it has better efficacy and safety,and is worthy of clinical application. |
