| Objective:to observe the effect of Yiqi Yangyin Huoxue Recipe combined with Danshen injection in the treatment of patients with Stage III Diabetic Kidney Disease syndrome of deficiency of both Qi and Yin combined with blood stasis,before and after the treatment,the changes of Vmax,Vmin,RI,renal function,urinary protein and other related indexes and symptoms were observed,to explore the effect of Yiqi Yangyin huoxue recipe on kidney Hemodynamics in DKD patients,so as to give full play to the advantages of traditional Chinese medicine in treating DKD,and to provide the basis for the early treatment of diabetic nephropathy.Methods:This study was conducted from October 2018 to October 2020 in-patient Department of Huayuan Mountain Hospital of Hubei Provincial Hospital of traditional Chinese medicine.The diagnosis of Western medicine was stage III diabetic nephropathy,the Syndrome differentiation of TCM is "Xiaoke nephropathy,deficiency of both Qi and Yin and blood stasis syndrome",60 cases in total.The 60 patients were randomly divided into treatment group(30 cases)and Control Group(30 cases).The Control Group was given life style intervention,Western medicine basic drug treatment(lowering blood pressure,lowering blood sugar,regulating blood fat)and Salvia injection intravenous treatment.The study was conducted over a two-week period,to observe the improvement of TCM symptoms,the clinical curative effect and the changes of both renal segmental artery Hemodynamics(Vmax,Vmin,RI),renal volume,urinary protein(UACR,24h-UP),Coagulation function(FIB、D-dimer),etc.,and to collect and sort out the above data,carry on the relevant statistical analysis,in order to evaluate the curative effect of this study.Results:1.Clinical Disease Curative Effect:The total effective rate of the treatment group was 80.0%,which was higher than that of the observation group(63.3%,P<0.05).2.The total effective rate of the treatment group was 90.0%,which was higher than that of the Observation Group(66.7%,P<0.01),the difference was statistically significant(P<0.01),and the decrease of integral score in the treatment group was better than that in the control group(P<0.05)Single score of TCM:The improvement of some single symptoms in the Treatment Group(Polyphagia,thirst,thirst,tiredness,fatigue,hyperhidrosis,turbid urine,frequent urination,five irritations,Edema,limb numbness and skin nail error)was significantly better than that in the control group,the difference was statistically significant(P<0.05).There was no significant difference in the improvement of some symptoms(lumbar soreness and weakness,constipation,insomnia).3.Changes of renal hemodynamics:Vmax and Vmin of bilateral segmental arteries were increased and Ri were decreased in both groups after treatment(P<0.05),which suggested that the renal Hemodynamics were improved in both groups,the decrease ratio of the treatment group was better than that of the control group(P<0.05),which suggested that Yiqi Yangyin huoxue prescription could increase the blood flow velocity of renal segmental artery and decrease the blood flow resistance.4.Renal Volume:There was no significant difference between the two groups(P>0.05)and there was no significant difference between the two groups(P>0.05),it is suggested that the two therapeutic schemes have little effect on reversion of renal enlargement.5.Urinary Protein Related Indexes:24 hours after treatment,up and EACR decreased in both groups(P<0.05),suggesting that the two groups can reduce urinary protein excretion,and the treatment group decreased more than the control group(P<0.05),it is suggested that Yiqi Yangyin huoxue prescription is more effective in reducing urinary protein excretion.6.Coagulation function:the FIB and D-dimer of the two groups were decreased after treatment(P<0.05),suggesting that the two groups had the effect of reducing blood viscosity,the decrease degree of the treatment group was better than that of the treatment group(P<0.05),which suggested that the prescription of supplementing Qi,nourishing Yin and activating blood could improve the glomerular microcirculation and protect the early renal function of DKD.Conclusion:The combination of Yiqi Yangyin Huoxue decoction and Danshen injection can improve the clinical symptoms of TCM,improve the kidney Hemodynamics,reduce the blood viscosity and improve the glomerular microcirculation in the patients with the syndrome of deficiency of both Qi and Yin and blood stasis in Stage Ⅲ diabetic nephropathy,reduce the excretion of Proteinuria,so on the early protection of renal function,but the role of reversing the size of the kidney is not obvious.In a word,Yiqi Yangyin huoxue prescription can delay the development of DKD and improve the quality of life of patients. |
