Clinical Observation On The Treatment Of Menstrual Irregularity Of Systemic Lupus Erythematosus By Invigorating Spleen And Removing Blood Stasis

Objective: To observe the efficacy and safety of "Jianpi Tiaojing Decoction" in the treatment of menstrual irregularity of SLE with spleen deficiency and blood stasis.To provide safe and effective TCM treatment methods for patients with systemic lupus erythematosus complicated with menstrual irregularity,so as to better play the advantages of TCM in the treatment of menstrual irregularity of systemic lupus erythematosus.Methods: A total of 60 patients with irregular menstruation of spleen deficiency and blood stasis type SLE in remission period diagnosed in Wuhan Hospital of Integrated Traditional Chinese and Western Medicine from May 2019 to December 2020 were collected and randomly divided into treatment group(n = 30)and control group(n = 30).After 3 menstrual cycles of treatment,relevant score indexes were recorded and analyzed statistically.All the enrolled patients maintained their original medication regimen in the treatment of SLE,the control group was treated with Western medicine based on this,and the experimental group was additionally treated with Jianpi Tiaojing prescription(basic prescription: Red Astragalus 10 g,Peony bark 15 g,Yujin 10 g,cassia twig 6g,Chuanxiong 10 g,red and white peony 10 g each,Bupleurum 10 g,Dangshen 10 g,Wuzhu 6g,Angelica 10 g,Stir-fried Six Divine Comedy10 g,Jiao Shu 10 g,plus and subtracting with the syndrome)Chinese medicine decoction is prepared and decocted by the pharmacy of Wuhan Integrated Traditional and Western Medicine Hospital,take three times a day,200 ml each time.Take it for 3 consecutive menstrual cycles.TCM clinical symptom score,menstrual symptom score,SLEDAI score,Hamilton HAMA anxiety score,C3,C4,ESR,CRP indexes and biochemical indexes to judge the safety of medication were observed and recorded respectively before and after treatment.SPSS25.0 software was used for statistical analysis of the data and the conclusion was drawn.Results:No case of abscission was found in 60 patients after treatment.Compared with the control group,30 cases in the treatment group were treated with Jianpi Tiaojing Decoction,of which 26 cases were effective and 4 cases were ineffective.The total effective rate was 86.67%.In the control group of 30 cases,19 cases were effective,11 cases were ineffective,the effective rate was 63.33%,the difference between the two was statistically significant,indicating that the treatment group had more obvious curative effect.In terms of TCM syndrome scores,the scores of each group decreased after treatment,and there were significant differences between treatment group and control group before and after the trial.There was no statistical difference between the groups before the experiment,but the difference was statistically significant after the experiment.The results indicated that the therapeutic group was better than the control group in improving TCM symptoms.In terms of SLEDAI score,scores of both groups decreased before and after treatment,and the difference was statistically significant.There was no significant difference between the control group and the treatment group before treatment,but the difference was significant after treatment.These results indicated that both the treatment group and the control group could reduce the SLEDAI score,and the treatment group had a better effect than the control group.In terms of Hamilton HAMA anxiety score,HAMA score decreased before and after treatment in the two groups,and the difference was statistically significant.There was no significant difference between the treatment group and the control group before treatment,but the difference was significant after treatment.It indicates that both groups can relieve the anxiety of patients,and the treatment group has a better effect.In terms of changes in laboratory indicators,there were statistical differences in C3,ESR and CRP indexes between the two groups before and after treatment.There was significant difference in C4 index in the control group before and after treatment,but no significant difference in the treatment group before and after treatment.It indicates that both groups can enhance immunity and reduce inflammation.Safety indicators: all the patients were in good condition in general,and there was no obvious abnormality in blood routine,liver and kidney function before and after treatment.Conclusion: Jianpitiaojing decoction can reduce the TCM syndrome score,SLEDAI score,HAMA anxiety score,ESR and CRP indexes,and can increase the complement C3.It has no significant effect on the liver and kidney function,blood routine and blood coagulation function of the patients,and has no allergy and gastrointestinal stimulation reaction,which is safe and good.Jianpitiaojing decoction can effectively improve the symptoms of irregular menstruation of SLE with spleen deficiency and blood stasis,stabilize the condition of SLE,relieve the anxiety of patients and improve immunity.
