Objective:To explore the current situation of postoperative fatigue(POF)in patients with coronary artery bypass grafting(CABG)and its influencing factors.To identify patients with high-risk of POF and to provide a basis for further preventive intervention measures,it is helpful to optimize the management of CABG and to improve the prognosis of patients.Methods:This study is a cross-sectional study.312 patients after coronary artery bypass grafting surgery from January to August 2020 in a tertiary Class A general hospital were selected as study subjects.Postoperative fatigue assessed by the Chinese version of multidimensional fatigue symptom inventory-profile(MFSI-SF-C)and data were collected using self-designed questionnaire,hospital anxiety depression scale(HADS),pain scale FPS-R,NRS2002 nutritional risk screening scale(2008),understanding social support scale(PSSS)and Pittsburgh sleep quality scale(PSQI)as a research tool for cross-sectional survey,using SPSS23.0 statistical software,using independent sample t test,analysis of variance,spearson correlation analysis and linear regression analysis the influence factors of selected patients with postoperative fatigue.The Mann-Whitney U test in nonparametric test was used to analyze the effects of high and low POF scores on the prognostic indicators such as early bed movement time and total hospital stay of CABG patients.Results:1.312 patients with coronary artery bypass surgery fatigue score was 22.67±14.25,including general fatigue score was 10.89±3.13,body fatigue score was 10.59±3.14,emotional fatigue score was 8.64 ± 4.24,mental fatigue score was 3.89 ±3.47,activity dimension score was 11.33± 3.69.2.According to independent sample t test or variance analysis,the differences of POF of CABG patients in age,body mass index,educational level,postoperative use of balloon counterpulsation,preoperative fatigue degree,intubation time and stay time in the intensive care unit were statistically significant(P<0.05).3.According to Pearson correlation analysis,POF was positively correlated with pain degree(r=0.312,P<0.001),nutritional risk score(r=0.346,P<0.001),sleep quality(r=0.328,P<0.001),anxiety(r=0.749,P<0.001),depression(r=0.730,P<0.001),and POF was negatively correlated with social support score(r=-0.362,P<0.001).4.The results of Logistic regression showed that:age,culture level,stay time in ICU,anxiety and depression,pain,nutritional risk screening score,the sleep quality were independent risk factors for POF in CABG patients5.The POF scores of CABG patients were divided into low group and high group.The Mann-Whitney U test in nonparametric test was used to compare the early time of getting out of bed between high group and low group of CABG patients(Z=-8.380,P<0.001),the total number of days of hospitalization(Z=-2.227,P=0.026)were statistically significant.The early time of getting out of bed in the high POF group was 7.00(6.00,8.00)days after operation,and the total hospital stay was 26.00(20.00,34.00)days,while the early time of getting out of bed in the low POF group was 6.00(5.00,6.00)days after operation,and the total hospital stay was 24.00(19.00,30.00)days.6.The time of early getting out of bed and total days of hospitalization were positively correlated with the total score of POF and one or more of the dimensions of general fatigue,physical fatigue,emotional fatigue and psychological fatigue in POF(r>0,P<0.05),and negatively correlated with the latitude of activity in POF(r<0,P<0.05).Conclusions:1.The postoperative fatigue scores of patients undergoing coronary artery bypass grafting are moderate,which should be of concern.POF involves multiple dimensions of body,physiology,emotion,etc.,and multi-dimensional comprehensive intervention should be carried out according to patients’ body,physiology,emotion and other aspects.2.The occurrence of postoperative fatigue is affected by age,education level,stay time in the care unit,pain,nutritional risk score,sleep quality,and postoperative anxiety and depression.It can reduce the stay time in the care unit,relieve the pain of patients,increase nutrition,improve sleep,reduce anxiety and depression to relieve postoperative fatigue.3.The postoperative fatigue,general fatigue,physical fatigue,emotional fatigue,and psychological fatigue were positively correlated with the early time of getting out of bed and the length of hospital stay,which affected the short-term prognosis of patients.It suggests that we should conduct multi-dimensional intervention and management on the physical,emotional and psychological aspects of postoperative fatigue,so as to reduce postoperative fatigue,improve the prognosis of patients and promote postoperative recovery.
