| Objective: To investigate the effects of immunophenotypes(CD117,CD34,Ki67)and pathological characteristics on prognosis survival rate and recurrence rate in patients with gastrointestinal stromal tumors(GIST),and to provide evidence and guidance for the clinical prognosis of patients with this disease.Methods: A total of 70 patients with stromal tumors diagnosed by postoperative pathology in the First Affiliated Hospital of Hebei North University from September 2015 to September 2016 were selected as the research subjects.The pathological characteristics and prognosis of 70 patients were analyzed retrospectively.The Kaplan-Meier method(Log-rank test)was conducted for univariate survival analysis.Cox regression model was used to analyze the correlation among immunophenotype,pathological characteristics and prognosis of GIST patients.Results:(1)Tumor size: The cases of the tumor size in the order of ≤2cm,>2cm≤5cm,> 5cm≤10cm,> 10 cm were 5,35,25,and 5,accounting for 7.1%,50.0%,35.8%,7.1%,respectively.Among them,the constituent ratio of the patients with the tumor size of > 2cm≤5cm was the highest.(2)Tumor location: Among 70 patients with GIST,43 tumors occurred in the stomach,accounting for 61.4%,21 cases were in the small intestine,accounting for 30.0%,and 6 cases were located in other sites,accounting for 8.6%.The main incidence location of GIST was the stomach.(3)Mitotic number: Among the 70 patients with GIST,there were 42,11 and 17 patients whose mitotic number was <5/50 HPF,≥5 ≤ 10/50 HPF,and> 10/50 HPF,,respectively.They accounted for 60.0%、 15.7% and24.3%,respectively.The majority of patients with GIST had mitosis of<5/50 HPF.(4)Risk grade: Fletcher classification of 70 patients with GIST was mainly moderate risk(45.8%)and high risk(25.7%).(5)The positive rates of immunological indexes in patients with GIST were in the order of CD117(95.7%),CD34(78.6%),and Ki67(57.1%).(6)The 3-year survival rate of the patients was 92.9%,and the 3-year recurrence rate of the patients was 28.6%.(7)The results of univariate analysis showed that the factors affecting the survival rate of patients with GIST included tumor size,tumor location,mitotic number,Fletcher grade and Ki67(P <0.05).(8)Cox regression model showed that the independent factors affecting the survival rate of patients with GIST included tumor size,mitotic number,and Fletcher grade(P <0.05).(9)The results of univariate analysis showed that the factors affecting the recurrence rate of patients with GIST included tumor size,tumor location,mitotic number,Fletcher grade,and Ki67(P <0.05).(10)Cox regression model showed that the independent factors affecting the recurrence rate of GIST patients included the mitotic number,Fletcher grade,and Ki67(P <0.05).Conclusion:(1)The positive expression rate of CD117 and CD34 in immunohistochemistry is high,which is an important indicator for the diagnosis of GIST.(2)The independent factors affecting the survival rate of patients with GIST include tumor size,mitotic number,and Fletcher grade.(3)The independent factors affecting the recurrence rate of patientswith GIST include the mitotic number,Fletcher grade,and Ki67. |
