| Pulmonary fibrosis(PF)is a common outcome of interstitial lung disease caused by multiple etiologies.At present,there are no clear treatment drugs and methods.Studies have found that angiogenesis and remodeling are the keys to the progression of PF.Clinical application of traditional Chinese medicine to treat PF has certain advantages.The pathogenesis of PF is mainly qi deficiency and blood stasis.Based on this design,this experiment is divided into two parts,clinical and experimental research.1 Clinical researchObjective:To observe the clinical efficacy and safety of Jiawei Buyang Huanwu Decoction in the treatment of PF,and to explore its treatment mechanism.Methods:54 IPF patients who met the inclusion criteria were randomly divided into observation group and control group.All patients were treated with basic western medicine.The control group was supplemented with Buyang Huanwu Decoction.The observation group was supplemented with Jiawei Buyang Decoction.The course of treatment was 3 months.The clinical symptoms,TCM syndrome scores,6MW test were observed before and after administration.Manage and evaluate the effect.Results:The total effective rate in the control group was 68%,and the total effective rate in the observation group was 84%.The TCM syndrome score and 6MW test were improved compared with those before treatment.The improvement degree of the observation group was better than that of the control group(P<0.05).Conclusion:The addition of Yiqiwenyanghuoxue method on the basis of conventional treatment can effectively improve the clinical symptoms of patients,and the addition of Wenyang method is better than that of Yiqihuoxue method alone.2 Experimental researchObjective:To observe the effect of Jiawei Buyang Huanwu Decoction on the expression of pigment epithelium-derived factor(PEDF)and endostatin in rats with pulmonary fibrosis induced by bleomycin,and to study its mechanism of action.Methods:60 healthy SD rats were randomly divided into five groups:blank group(N),model group(M),Buyang Huanwu decoction group(A),Jiawei Buyang Huanwu decoction(B),Prednisone group(C),except for the N group,I used bleomycin tracheal instillation for pulmonary fibrosis modeling and experimental drugs for intragastric administration for 28 consecutive days.On day 14,3 samples were sacrificed in each group.The remaining samples were sacrificed in each group on day28.The lung tissue of rats was observed under a light microscope,and the expressions of PEDF,endostatin gene and protein in each group were detected by PCR and Western blotting.Results:After drug intervention,the expressions of PEDF,endostatin mRNA and protein in groups A,B,and C were significantly higher than those in group M,and group B was significantly better than those in groups A and C(P<0.05).Conclusion:The treatment of lung fibrosis with Yiwenyangyanghuoxue method may be related to increasing PEDF and endostatin levels and inhibiting angiogenesis. |
