Correlation Between P Wave Dispersion,QTc Dispersion,Blood Pressure Variability And Hypertensive Disorder Complicating Pregnancy

Objectives: Hypertensive disorder complicating pregnancy(HDCP)is a special type of hypertension,which is one of the serious complications in pregnancy.It affects the health of pregnant women and fetus seriously.It also increases the risk of hypertension,cardiovascular disease and death in the rest of the mother’s life.To analyze the relationship between pregnancy induced hypertension and P wave dispersion,QTc interval dispersion and blood pressure variability.Methods: From January 1,2017 to December 31,2019,213 pregnant women who met the diagnosis of pregnancy induced hypertension were selected as the group of pregnancy induced hypertension,and 248 healthy pregnant women who were delivered successfully in hospital at the same time were selected as the control group.The inclusion criteria of the two groups: single pregnancy,age 18-45 years old,excluding the previous history of polycystic ovary syndrome,thyroid disease,chronic kidney disease,lupus erythematosus,rheumatoid,diabetes,gestational nephropathy,no need for long-term medication and multiple pregnancy of pregnant women,no mental state abnormality or mental history.The observation indexes included age,body mass index(BMI),gestational age,admission systolic blood pressure,diastolic blood pressure,P wave minimum time(Pmin),P wave maximum time(Pmax),P wave dispersion(PD),QTc minimum time(qtmin),QTc maximum time(qtmax),QTc dispersion(QTd).The blood pressure variability between the treatment group and the non treatment group,the severe eclampsia group and the eclampsia group were compared.The measurement data are in accordance with normal distribution,and the results are expressed by means of mean ± standard deviation.The t-test was used for the comparison between groups.The binary logistic regression model was used to evaluate the effects of gestational weeks,BMI,P-wave minimum,P-wave dispersion,QTc minimum time and QTc dispersion on the study of pregnancy induced hypertension.The difference was statistically significant(P < 0.05).Results: 1.There was no significant difference in age between the HDCP group and the control group.The gestational age of the HDCP group(37.53 ± 2.60)was significantly lower than that of the control group(39.31 ± 2.64),P<0.05.BMI(28.58 ± 5.19)kg / m2 in HDCP group was significantly higher than that in control group(25.48 ± 2.72),P<0.05.2.The minimum time of P wave(78.59 ± 9.32)ms and QTc(384.65 ± 21.69)ms in HDCP group were significantly lower than those in control group(94.61 ±7.03)ms and QTc(401.91±15.4).The dispersion of P wave(42.75 ± 9.94)ms and QTc(57.15 ± 16.10)in HDCP group were significantly higher than those in control group(14.91 ± 4.03)ms and QTc(21.42 ± 6.07),(P < 0.05).There was no significant difference in the maximum time of P wave and QTc between the two groups.3.According to the use of hypertensives,the patients were divided into medication group and non medication group.There was no significant difference in systolic blood pressure variability between the treatment group and the control group.Diastolic blood pressure variability(0.123 ± 0.031)in the control group was significantly higher than that in the control group(0.113 ± 0.29),P < 0.05.There was no significant difference in diastolic blood pressure variability between the eclampsia group and the severe eclampsia group.The systolic blood pressure variability(0.105 ± 0.083)in the severe eclampsia group was significantly higher than that in the eclampsia group(0.086 ± 0.025),P < 0.05.4.The effects of gestational weeks,BMI,P wave minimum,P wave dispersion,QTc minimum time and QTc dispersion on hypertensive disorders complicating pregnancy were evaluated by binary logistic regression.P wave dispersion(OR = 1.795,95% CI 1.266-2.546),QTc minimum time(OR = 0.017,95% CI 0.833-0.983),and QTc dispersion(OR = 1.216,95% CI 1.042-1.420)were statistically significant.Conclusion: P wave dispersion and QTc dispersion were significantly increased in patients with hypertension disorders complicating pregnancy,and systolic blood pressure variability in severe eclampsia was higher than that in mild eclampsia,The use of antihypertensive drugs can reduce the variability of diastolic blood pressure in patients with pregnancy induced hypertension.This study suggests that P-wave dispersion,QTc dispersion and blood pressure variability are related to pregnancy induced hypertension.Further study is needed to find out its clinical significance.
