| Objective:Retrospective analysis of the monocentric data of hospitalized patients with psoriasis,learning the comorbidities and clinical features.Improving acknowledgement of psoriasis comorbidities for clinician,and providing guidelines and assistance for the integrated treatment of psoriasis.Methods: The clinical data of patients admitted in the First Affiliated Hospital of China Medical University from January 2004 to December 2018 were collected,including gender,age of onset,clinical type,place of residence,history of smoking,history of alcohol abuse,comorbidities,laboratory inspection,etc.Using SPSS 22.0 to describe and analyze data,and P < 0.05 was considered that difference between the two groups has statistically significance.Results:There are 914 cases collected in the study,a total of 52 patients aged <18 years old,including psoriasis vulgaris 35 cases,pustular psoriasis 13 cases,and erythrodermic psoriasis 4 cases.The proportion of comorbidities in the juvenile group is significantly less than that in the adult group,including hyperlipidemia 2 cases,digestive tract diseases 1 case,and connective tissue disease 1 case.In the adult group,psoriasis vulgaris is the most common,with a total of 535 cases(62.06%),other types of psoriasis are 327 in total,including erythrodermic psoriasis188 cases(21.81%),pustular psoriasis 103 cases(11.95%),and arthritic psoriasis 36cases(4.18%).The top three comorbidities in the adult group are hyperlipidemia,hypertension and diabetes,respectively 20.07%,19.37% and 12.53%,followed by metabolic syndrome(6.96%),liver disease(6.38%),cardiovascular disease(5.57%),infection(3.71%),kidney diseases(3.02%),hypoproteinemia(2.90%),stroke(2.78%),autoimmune disease(2.78%).In addition,there are comorbidities such as malignant tumors,atopic disease,hyperuricemia,upper digestive tract diseases,2.78%、1.62%、1.39% and 1.16% respectively.Smoking and drinking are significant risk factors for hypertension in psoriasis patients.Relatively,male patients are more susceptable to hyperlipidemia,hyperuricemia and upper digestive tract diseases than female patients,and the later have higher risks of infection and kidney disease.Arthritic psoriasis patients are more susceptible to autoimmune disease than other types patients.Erythrodermic psoriasis patients and pustular psoriasis patients are more likely to get hypoproteinemia.The age of psoriasis onset in patients with hypertension,diabetes,cardiovascular disease,stroke,autoimmune disease,hypoproteinemia,kidney disease,and malignant tumors was higher than those without corresponding diseases.Patients from cities are mostly psoriasis vulgaris,instead,the percentage of patients of other types from villages are larger than that from cities.It is significantly different in the age of onset between group with and without comorbidities,the former is much more higher(P<0.05).Conclusion:1.The hospitalized patients in our study are mostly medium-ederly men,late-onset cases are common.Most patients with psoriasis are accompanied by hyperlipidemia,hypertension,diabetes and metabolic syndrome.2.Smoking and alcohol abuse could increase the risk of hypertension.3.The types of comorbidities in psoriasis patients could be influenced by gender.4.Patients who have relative later onset always have comorbidities.5.It is necessary to prevent comobidities which may occur when treating psoriasis.Active treatment for primary comorbidities could reduce psoriasis severity. |
