| Objective:To analyze the correlations between the morphology of pterygium and corneal aberrations,and to see if the postoperative corneal higher-order aberrations for one month and preoperative corneal higher-order aberrations are linked,to investigate the optimal time for pterygium surgery.Methods:This study is a prospective study.42 primary patients(45 eyes)who underwent pterygium excision and 21 patients(24 eyes)who were followed up in one month in the Department of Ophthalmology of the First Affiliated Hospital of Guangxi Medical University from July 2019 to January 2020 were observed.Unilateral patients were involved.The higher-order aberrations and corneal astigmatism were measured by Oculus of Germany.Meanwile,The shape and size of pterygium in patients were measured by angle gauge,including pterygium extension(distance from the middle to the top of limbal)to the apex of the pterygium head),width(chord extension of pterygium on the limbal),total area(calculated according to extension and width:(extension x width/2).All of them accept pterygium excision and conjunctival autograft transplantation under local infiltration anesthesia,All above objects were operated by one skilled surgeon,and followed up the corneal aberration and astigmatism results of the eyes for one month after surgery.To investigate the correlations between the size of pterygium and preoperative corneal astigmatism,preoperative higher-order aberrations(include spherical aberration,vertical coma,horizontal coma,horizontal clover and oblique clover).And knew about the link between preoperative higher-order aberrations and postoperative higher-order aberrations for one month.The discrepancy was statistically significant with P<0.05.Results:The extension of pterygium is 1-7mm,the mean of it was 3.67?1.26mm,the width is 2-8mm,ranged with a mean of 5.66?1.37mm,the total area is 1-28mm~2,with a mean of 10.91?5.44mm~2.Pearson linear correlation analysis showed that all variables(length,width and area)of pterygium are significantly positively correlated with the preoperated total higher order aberrations.The correlation values are 0.731,0.626 and 0.754 respectively,The above three values were statistically significant(P<0.05).The extention,width and total area of pterygium have obvious positive correlation with horizontal coma and horizontal clover(P<0.001),while the correlation between the width of pterygium and corneal spherical aberration has no statistical significance.The results of multiple linear regression showed that the total area of pterygium has a stronger correlation with the preoperative higher order aberrations.(P<0.05).The extension,width and total area of pterygium are significantly positively correlated with corneal astigmatism,with respective r=0.661,r=0.466,r=0.64,which are statistically significant(P<0.001).The results of multiple linear regression showed that the total area of pterygium has a stronger correlation with the corneal astigmatism.(P<0.05).The preoperative total higher order aberrations of 21 patients(24 eyes)was4.26?3.09μm and the postoperative total higher order aberrations was1.35?0.98μm,the analysis showed that the preoperative higher order aberrations were positively correlated with the postoperative total higher order aberrations for one month,the correlation value is 0.452,with statistical significance(P<0.05).The preoperative horizontal coma and preoperative horizontal clover were positively correlated with the postoperative total higher order,and their correlation values:r=0.449,r=0.442,with respective p=0.028 p=0.031,the above two values were statistically significant(P<0.05).Conclusion:1.The extention,width and total area of pterygium are significantly positively correlated with the preoperative total coneal higher order aberration,horizontal coma and horizontal clover,and the total area of pterygium is the biggest influence factor on the the preoperative total coneal higher order aberration.2.The extention,width and total area of pterygium are positively correlated with preoperative corneal astigmatism,and the total area of pterygium is the biggest influence factor on the preoperative corneal astigmatism.3.The total postoperative higher-order aberrations of pterygium for one month are positively correlated with the total preoperative higher-order aberrations、preoperative horizontal coma and preoperative horizontal clover. |
