| ObjectiveTo investigate the clinical value of MRI multi-b value diffusion weighted imaging Multi-b DWI(Mb-dwi)and apparent diffusion coefficient(ADC)in the diagnosis of transition zone prostate cancer(TZ-Pca)and bengin prostate hyperlasis(TZ-BPH.)MethodsA retrospective analysis was performed on 130 patients who underwent routine MRI and MB-DWI examination of prostate in our department from January 2018 to October 2019,including 60 patients with transitional prostate cancer(TZ-Pca group)and 70 patients with benign prostatic hyperplasia(TZ-BPH group).The difference of ADC value and signal strength between the two groups with different b values was statistically analyzed and compared with the "gold standard" of pathological examination.By two experienced doctors according to the present standard MRI imaging in the diagnosis of prostate cancer in a double-blind reader,to make the determination of the cancer foci and non-cancer foci,the result and prostate surgery or biopsy pathology results comparison,the decision matrix,respectively,to verify with single value of DWI(Single-b-DWI,SB-DWI)cancan b(b = 1000 s/mm squared)combined with ADC value and b value of DWI(MB-DWI)can combination of ADC values between diagnostic performance,and the four data table chi-square test,and based on postoperative pathology of prostate,further analyzing the sensitivity and specificity of prostate cancer in both groups,P < 0.01 indicates that the difference is statistically significant.ResultsWhen b value =400s/mm,600s/mm,1000s/mm,1500s/mm,1500s/mm,1500s/mm,ADC values of TZ-PCa group were(0.91± 0.23,0.83± 0.19,0.72±0.14,0.66 ± 0.15,0.61 ± 0.09)significantly lower than that of TZ-BPH group(1.42 ± 0.21,1.19 ± 0.33,1.01 ± 0.18,0.97 ± 0.22,0.85 ± 0.10),and the differences were statistically significant(P<0.05).The DWI signal strength of TZ-PCa group was higher than that of TZ-BPH group under different b values,and the difference was statistically significant(P<0.05).The slopeb-signa in TZ-PCa and TZ-BPH groups were 63.46 ± 18.96 and 72.73 ± 21.25,respectively,and TZ-PCa group was lower than TZ-BPH group.The Sde% in the two groups was 62.45+13.44 and 70.89 ± 12.88,respectively.The TZ-PCa group was lower than TZ-BPH group,and the difference between the two groups was statistically significant(P<0.05).The sensitivity to the diagnosis of prostate cancer in the single b value DWI binding ADC group and the multi-b value DWI binding ADC group was 71.66% and 88.33%,respectively,with no significant differences(P > 0.05).The specificity was 42.85% and 91.43%,respectively,with statistically significant differences(P < 0.05).ConclusionMRI MB-DWI combined with ADC value has statistically significant diagnostic value in the diagnosis of transitional prostate cancer,and the diagnostic efficiency of MB-DWI combined with ADC value is significantly improved. |
