| Objective:Two-dimensional speckle tracking imaging(2D-STI)was used to measure the overall longitudinal strain(LAS)of the left atrial and the mitral annular displacement(MAD)in patients with atrial fibrillation to evaluate the left cardiac function.At the same time,the changes of both were tracked after catheter ablation to evaluate the effect of catheter ablation on left cardiac function in patients with atrial fibrillation.Method:Routine echocardiography is required for all subjects.Apex four-chamber heart rate(AP4-HR),left atrial strain of reservoir(LASr),left atrial strain of conduit(LAScd),left atrial strain of contraction(LASct),maximum displacement at septal side of mitral annular(MAD 1),maximum displacement at lateral side of mitral annular(MAD2),maximum displacement at the midpoint of the line between septal side and lateral side(MADmidpt)and maximum displacement at the midpoint of the line between septal side and lateral side which was the percentage of the left ventricular length diameter(MADmidpt%)were measured.In the first part,Patients with atrial fibrillation before catheter ablation were divided into group A(persistent and long-term persistent atrial fibrillation)and group B(paroxysmal atrial fibrillation).Meanwhile,normal people with age and gender matching were selected as group C.Differences in left atrial strain and mitral annular displacement were compared among groups.In the second part,Patients with atrial fibrillation who were followed up for 3 months after catheter ablation were divided into group M(persistent and long-term persistent atrial fibrillation)and group N(paroxysmal atrial fibrillation),and the differences of left atrial strain and mitral annular displacement before and after catheter ablation were compared among the groups.Results:In the first part,44 patients were enrolled in group A,30 patients in group B and 35 patients in group C.①Group A and group B were compared.LASr,LAScd,MAD1,MAD2,MADmidpt and MADmidpt%were lower than those in group B(P<0.05);②Group A and group B were compared with group C.LASr,LAScd,LASct,MAD1,MAD2,MADmidpt and MADmidpt%in groups A and B were lower than those in group C(P<0.05);③MAD in atrial fibrillation group were negatively correlated with AP4-HR.In the second part,15 patients were enrolled in group M,8 patients in group N,and they were all sinus rhythm.LASr,MAD2,MADmidpt and MADmidpt%increased after catheter ablation in group M and N(P<0.05);MAD1 in group M and LAScd in group N increased after catheter ablationn(P<0.05).Conclusion:Atrial fibrillation may reduce atrial storage,piping,pumping function and ventricular systolic function.Compared with LVEF,mitral annular displacement can reflect changes in left ventricular systolic function earlier.Catheter ablation can improve atrial storage,piping,pumping function and ventricular systolic function in patients with atrial fibrillation. |
