Objective:To explore the changes of serum levels of myelin related proteins and the preliminary pathological mechanism of myelin in Early Onset Schizophrenia(EOS)patients.Meanwhile finding serum biomarkers and establish a diagnostic model for EOS.Methods:30 EOS children were selected as the EOS group,and 30 normal developing children matching their age and sex ratio were selected as the healthy controls(CT).The following methods were used to study:(1)Serum concentrations of myelin related factors were detected by ELISA:myelin basic protein(MBP),myelin oligodendrocyte glycoprotein(MOG),myelin associated glycoprotein(MAG);brain derived neurotrophic factor(BDNF),nerve growth factor(NGF),soluble protein(S100β),glial fibrous acidic protein(GFAP);tumor necrosis factor-alpha(TNF-α)and interleukin-6(IL-6).Patients in EOS group were assessed by positive and negative syndrome scale(PANSS)and brief psychiatric rating scale(BPRS);(2)Spearman was used to analyze the correlation between myelin specific protein and other protein factors in EOS;(3)Spearman analyzed the correlation between each of protein factors and PANSS and BPRS scores,and corrected by False Discovery Rate(FDR);(4)Logistic regression analysis was used to sift indicators with high diagnostic value for EOS,and establishing a diagnostic model,using a subject operating characteristic curve to evaluate the diagnostic value of this model.Results:(1)MBP(1268.0(961.2)ng/L)serum concentrations in EOS were higher than that in CT(627.0(868.9)ng/L),MAG(485.9(87.4)ng/L),BDNF(1900.0(322.0)ng/L),NGF(3730.0(598.0)ng/L),S100β(197.2(118.5)ng/L),GFAP(537.4(184.8)ng/L)were lower than that in CT(604.4(242.2),3050.0(1215.0),4462.0(1997.0),297.2(90.5),732.8(314.1)ng/L,respectively)(U=128.0,142.0,63.00,176.5,205.0,158.0,all P<0.01),and there was no statistically significant difference in serum concentration of MOG,TNF-α and IL-6(P>0.05);(2)MBP was positively correlated with S100β and GFAP(r=0.641,0.637,both P<0.01),MAG was positively correlated with BDNF and NGF(r=0.578,0.473,both P<0.05),and there was no significant correlation between residual protein factors(P>0.05);(3)After correction of FDR,there was no significant correlation between serum concentrations of each myelin related factor and EOS symptom score;(4)MBP(B=0.006,P=0.012)and BDNF(B=-0.004,P=0.004)were screened by Logistic regression analysis.The diagnostic model was Logistic(P)=4.545+0.006CMBP-0.004CBDNF,and C was the protein concentration in ng/L.The analysis of the receiver operating characteristic curve showed that the area under the curve of this model was 0.9659,the sensitivity was 91.67%,the specificity was 95.45%,the likelihood ratio was 20.17,and the accuracy was 91.3%.Conclusion:(1)Changes of serum myelin related protein levels suggest that patients with EOS may have myelin abnormalities.(2)The changes of MBP and MAG are respectively correlated with different types of proteins,suggesting that there may be different mechanism pathways for the changes of MBP and MAG.(3)The combination of MBP and BDNF may have certain accuracy in the diagnosis of EOS.
