| Objective: Postoperative pain of root canal treatment has always been a thorny problem in clinical work,and root canal preparation is one of the key steps in root canal therapy.In order to reducing the postoperative reaction of root canal treatment,the clinician should as far as possible to choose the appropriate root canal preparation equipment system in the process of root canal preparation.In order to guiding clinical,this study evaluated the postoperative pain、dentinal cracks、apically extruded debris and preparation time using reciprocating instrument(WaveOne)and the continuous rotary instruments(ProTaper)prepared root canal,comparing the clinical efficacy and security of two kinds of systems.Methods : Electronic retrieval as follows:Foreign language database: PubMed、The Cochrane Library 、EMBASE;Chinese database: Chinese journal full-text database(CNKI),wangfang database(wanfang data)and VIP database(VIP).According to the inclusion criteria and exclusion criteria,collected the randomized controlled trials(RCT)of postoperative pain using WaveOne reciprocating file instruments and The continuous rotary motion instruments ProTaper(including ProTaper Universal and ProTaper Next)prepared root canal;Experiment in vitro were collected to evaluate dentinal cracks and apically extruded debris of using WaveOne and ProTaper;And the review of postoperative pain、dentinal cracks、apically extruded debris practice using reciprocating motion system(WaveOne)and the continuous rotary motion system(ProTaper).Languages including English and debris Chinese,collect the date as of November 2017.According to the methods of Cochrane Reviewers’ Handbook recommend by The Cochrane Collaboration,meta-analysis was performed with Rev Man5.3 software.Results:1.According to computer retrieval,preliminary detected 236 literatures about postoperative pain 、 root mircocrack 、 apically extruded debris using reciprocating motion system(WaveOne)and the continuous rotary motion system(ProTaper).After reading the title,abstract and full text filtering process,26 literature were finally included.2.The results of meta-analysis showed that the total incidence of postoperative pain after root canal treatment have no significant difference RR = 0.97,95% CI(0.74,0.74),95% CI of the RR contains 1,the combined effect of inspection Z = 0.18,P = 0.86.The incidence of in mild pain,moderate pain and severe pain level have no significant difference RR were 0.86(0.55,0.55),1.23(0.59,0.59),0.73(0.26,0.26),the combined effect of inspection Z = 0.47,P = 0.64.3.Comparison of the numbers of teeth with root microcrack at the three sectioned levels(3mm、6mm 、 9mm)using rotary and reciprocating instuments.The results of meta-analysis showed that the numbers of teeth with root microcrack using reciprocating instuments WaveOne canal is lower than the ProTaper after root canal treatment RR were 0.81(0.47,0.47),0.69(0.45,0.45),0.56(0.40,0.40),combined effect quantity inspection Z = 2.94,P = 0.003.4.Comparison of apically extruded debris using rotary and reciprocating instuments.The results of meta-analysis showed that the amount of apically extruded debris using WaveOne was less than the continuous rotary motion instuments ProTaper,and have significant difference MD = 0.29,95% CI(0.83,0.83),combined effect quantity inspection Z = 1.05,P = 0.29.5.The results of meta-analysis show that the WaveOne has less time to prepare the root canal than the continuous rotation system ProTaper,and have significant difference MD = 109.77,95%CI(129.03,90.52).Conclusion:In this study,the results of meta-analysis show:1.The incidence of postoperative pain and was close to using reciprocating instruments WaveOne and continuous rotary motion instruments ProTaper.The postoperative pain difference have no statistically significant.RCT study with high quality about comparing WaveOne reciprocating file instruments and The continuous rotary motion instruments ProTaper is still needed.2.The time of the WaveOne preparation root canal is significantly less than that of the continuous rotary instruments ProTaper.3.The numbers of teeth with root microcrack using reciprocating instuments WaveOne canal is lower than the ProTaper after root canal treatment.3.The amount of apically extruded debris using continuous rotary motion instuments ProTaper was more than continuous rotary motion instuments ProTaper,and have significant difference.RCT study with high quality about comparing WaveOne reciprocating instruments and The continuous rotary motion instruments ProTaper is still needed.Through systematic evaluation of the incidence of postoperative pain and root microcrack,the amount of apically extruded debris and the preparation time,the results showed that the effectiveness of WaveOne preparation was better than that of ProTaper. |
