In-hospital disease spectrum is a comprehensive reflection of diseases that rank in accordance with varying degrees of harms to in-patients in a certain area.It reveals the basic conditions of in-patients and its changing trend.At present,the chronic non-communicable diseases(chronic diseases),which are mainly cardiovascular and cerebrovascular diseases,have become the leading death in residents of Jilin Province and Yanbian Korean Autonomous Prefecture,consuming a large amount of health resources.It is the most prominent public health issue.Thus,the work studied the changes in the disease spectrum of a secondary hospital in Yanbian Prefecture,and monitored the residents’ main diseases and the diseases of key in-patients.It is of great significance to establish scientific prevention and control measures for key diseases,and to reduce the economic burden of residents.Purpose1.Study the top 10 disease categories,the top 10 disease constituent ratio and the sequence of in-patients in a secondary hospital,analyzing the major diseases that threaten the health of residents.2.Study the constituent ratio and sequence of the top 10 disease system categories and the top 10 diseases causing death,analyzing the basic situation of diseases causing death.3.Study the basic status of hospitalization in different age groups in a secondary hospital.4.Study the basic conditions of hospitalization for different gender groups(the distribution of the top 10 male diseases as well as that of the top 10 female diseases).Methods1.Constituent ratio of disease spectrum and death:Also known as the constituent index,it indicates the proportion or distribution of a certain internal component occupies its whole.Usually 100%is taken as scale base.2.Data collection:A total of 32,674 medical records,as the subject,were collected from a secondary hospital from January 2012 to December 2015.The data in the work was mainly divided into institutional survey and routine statistics.Wherein,the first type of institutional survey came from the basic information(sex,age,etc.),case diagnosis of major diseases before hospitalization and discharging acquired in a hospital medical record system,as well as the information from the questionnaire survey for the medical institution.The second type was regular statistics,such as the Yanbian Health Statistics Statement(2012-2016),the Chinese Health Statistics Yearbook(2007-2011),and Yanbian Statistics Yearbook(2013-2016).3.The research method was classified according to the internationally accepted ICD-10 standard,with the data for sequence analysis.All data were analyzed using Excel software to analyze the sequence changes and distribution of the disease spectrum and the death spectrum of in-patients.Results1.In terms of class composition ration and sequence of disease system in a secondary hospital in Yanbian Prefecture,the sequence of the top 3 system disease from 2012-2015 is circulatory system,digestive system,injury and poisoning,and external causes.In addition,the sequence rose for the respiratory system,muscle,skeletal system and connective tissue diseases.2.Constituent ratio and sequence of the top 10 diseases in a secondary hospital in Yanbian PrefectureConstituent ratio and sequence analysis showed there was a significant change in sequence from 2012-2015.Cerebrovascular disease and ischemic heart disease always ranked the first and second.The sequence rose for chronic obstructive diseases(including liver disease,hypertension,diabetes,bronchitis,emphysema)and intervertebral disc diseases.3.Catalogue composition and sequence of death system for in-patients in a secondary hospital in Yanbian PrefectureAccording to the class composition and sequence of death of in-patients,the top 3 death system sequence from 2012 to 2015 was circulatory system disease,tumor and digestive system,accounting for more than 69%of the total death in the in-patient system.The most common death systems are circulatory diseases,tumors,digestive diseases,symptoms and signs,clinical and experimental abnormalities,and respiratory diseases.4.Constituent ratio and sequence of the top 10 diseases in a secondary hospital in Yanbian PrefectureFrom the specific death,from 2012 to 2015,the top 4 disease sequence was cerebrovascular disease,malignant tumor,ischemic heart disease,and liver disease,with no change in sequence during the period.The sequence rose for diabetes,hypertension,drugs and pharmaceuticals,and biological product poisoning.5.In-patients at different age groups in a secondary hospital in Yanbian PrefectureFrom the perspective of the age composition,the hospitalizations were mainly concentrated in people aged over 60 years from 2012 to 2015,accounting for 57.65%.The average number of in-patients aged 45 to 59 was 24.67%.The male hospitalization was mainly concentrated in people over the age of 60,accounting for 55.90%in 4 years.The hospitalization rate in 45-49 age groups was 28.85%,with increased hospitalized cases year by year.The female hospitalization was mainly concentrated in people over the age of 60,accounting for 59.33%in 4 years.The hospitalization rate in 45-49 age groups was 20.67%,with increased hospitalized cases year by year.5.In-patients in different gender groups in a secondary hospital in Yanbian PrefectureAccording to different sex populations in hospital,the total number of males in-patients from 2012 to 2015 was 15,973,accounting for 48.89%of the total number of hospitalizations.The number of female in-patients was 16,701,accounting for 51.11%of the total number of hospital admissions.The total number of female in-patients and the constituent ratio were higher than those of male in-patients during the four years.From 2012 to 2015,the top three diseases for male in-patients were cerebrovascular disease,ischemic heart disease and fractures,and the sequence kept unchanged compared with others that changed obviously.The top three diseases for female in-patients were cerebrovascular disease,ischemic heart disease,and hypertension,with sequence unchanged.In addition,the sequence has changed for other diseases instead of diabetes and liver disease in four years.Conclusions1.From 2012 to 2015,the sequence of the top 3 system diseases in a secondary hospital was circulatory system,digestive system diseases,injury and poisoning and external factors,with no change in sequence.In four years,the main hospitalized diseases in a secondary hospital were cerebrovascular disease,ischemic heart disease,malignant tumor,fracture,pneumonia,and diabetes.2.From 2012 to 2015,the sequence of death in a secondary hospital was circulatory diseases,tumors,and digestive diseases.In four years,the main death in a secondary hospital was cerebrovascular disease,malignant tumor,ischemic heart disease,liver disease,hypertension,and diabetes.3.From 2012 to 2015,the hospitalization was mainly concentrated in people over the age of 60,accounting for 57.65%of the total hospitalization.In the four years,the male and female in-patients were mainly concentrated in the aged over 60 years,accounted for 55.90%and 59.33%of the total hospitalization in four years,respectively.4.From 2012 to 2015,the total number and constituent ratio of female in a secondary hospital were higher than those of men.The main diseases of male in-patients are cerebrovascular disease,ischemic heart disease,fracture,liver disease,diabetes,and malignant tumor.The main diseases of female in-patients are cerebrovascular disease,ischemic heart disease,hypertension,fracture,obstruction,diabetes,and liver disease.
