Objective To observe the effect of ZUOGUEI Pill combined with stem cells transplantation on the renal pathologic changes and expression of PCNA/bFGF in rats model of focal segmental glomeruloslerosis(FSGS),providing an experimental basis for the clinical application of them and putting forward a new idea for the treatment of Chinese medicine and stem cell in chronic renal failure.Methods Separation and culture the cell suspension of rats’ BMSCs,the density of BMSCs is about 1*105 cells/cm2,and then identified the cells.30 female SD rats were randomized to normal group,model group,Chinese medicine group,stem cell group and stem cell combined with Chinese medicine group.Unilateral nephrectomy followed twice by intravenous injection of adriamycin at postoperative 7th and 28th day brought about typical manifestation of glomerulosclerosis on experimental rats.Stem cell group and stem cell combined with Chinese medicine group were injected with BMSCs through the tail vein at postoperative 35th day,and other groups were injected with saline solution.Chinese medicine group and stem cell combined with Chinese medicine group were filled the stomach with invigorating kidney for consolidating Semen Chinese medicine 1.0ml/100g*d at regular time,the rest groups with the amount of normal saline lavage.24-hours’ urinary protein were measured at 6th and 7th week.At 8th week all rats were blooded and then killed.Blood biochemical indicators were measured and the right kidneys were obtained for pathology observing.To observe the effect of the expression of PCNA/bFGF by immunohistochemistry in renal tissue,then semi-quantitatively analysed the results.Results(1)The successfully established of experimental model of FSGS.In eight weeks,compared with the normal group,model rats’ Scr,BUN have increased while Alb has decreased significantly(P<0.01).In the 6,7-week,compared with the normal group,model rats 24h UP has increased significantly(P<0.01).(2)The improvement of renal function.Each treatment group has different degrees of decline about Scr.Though in eight weeks,in treatment groups,Scr,Alb have no significant difference with the model group(P>0.05).Comparison of rats in the stem cell group and stem cell combined with Chinese medicine group,BUN were lower,there were significant difference between them(P<0.05).(2)(3)Improve the renal pathological.Renal pathology prompted varying degrees of glomerular sclerosis,moderate to severe hyperplasia of mesangial cells and matrix,tubular epithelial cells injury and interstitial atrophy with varying degrees of fibrosis in model group,after the treatment of Chinese medicine,stem cell or stem cell with Chinese medicine,renal fibrosis was reduced obviously.(4)The effect on the expression of PCNA/bFGF in each group.PCNA expressed little in normal kidney tissue,highly expressed in model renal tissue,compared with the model group,the expression of it was decreased in treatment groups,which was statistically significant in Chinese medicine group,stem cell group and stem cell combined with Chinese medicine group(P<0.05);bFGF expressed little in normal kidney tissue,highly expressed in model renal tissue.Compared with the model group,the expression of bFGF was increased in Chinese medicine group and stem cell group(P<0.05),while in stem cell combined with Chinese medicine group,bFGF has no significant difference with the model group(P>0.05).Conclusion 1.Unilateral nephrectomy plus points of the tail vein injection of doxorubicin can be provided a good experimental model of FSGS.2.Stem cell group and stem cell combined with Chinese medicine group can improve the renal function.Whether stem cell or Chinese medicine both can inhibit the expression levels of PCNA/bFGF in renal tissue of renal fibrosis rats,then delay renal fibrosis.
