Objective:To discuss the efficacy and safety of ultrasound-guided ilioinguinal-iliohypogastric nerve block in elderly patients undergoing inguinal hernia surgery.Methods:Patients undergoing inguinal hernia surgery were enrolled in this randomnized controlled study from Janurary to November 2018,which were devided into the control group(n=40)and the observation group(n=40)by random number table.Patients in control group were undergoing intraspinal anesthesia while patients in observation group were undergoing ultrasound-guided ilioinguinal-iliohypogastric nerve block.The two groups were then compared heart rate(HR),mean arterial pressure(MAP)and pulse oximetry(SPO2)in time entering the operation room(T0),5minutes after anesthesia(T1),after skin incision(T2),when surgery was finished(T3),12 hours after surgery(T4),24 hours after surgery(T5).The VAS score 12,24 hours after surgery,operation time,and postoperative recovery time,length of stay,postoperative degree of satisfaction upon anesthesia,and adverse reaction were also included.Results:The observation group had no significant difference in HR,MAP,SPO2in every time point(p>0.05).The HR and MAP of T1and T2in the control group were significantly lower compared with T0(p<0.05).The control group had significant lower and MAP compared with the observation group in T1 and T2(p<0.05).The VAS score had no significant difference between the two groups in T3and T5(p>0.05).The observation group had significantly lower VAS score than the control group in T4.The observation group had significant better operation time,postoperative recovery time and degree of satisfaction in anesthesia than the control group(p<0.05).The observation group had significantly lower rate of adverse reaction than the control group(12.5%vs 40%)(p<0.05).Conclusion:Ultrasound-guided ilioinguinal/iliohypogastric nerve block has better efficacy in elderly patients undergoing inguinal hernia surgery.It has less effect on the physiology of elderly patients and is safe to conduct.It is suitable for elderly patients undergoing inguinal hernia surgery who is not suitable for intraspinal anesthesia.
