| In China, the objective facts of large population, uneven distribution of medical resources between urban and rural areas, and inefficient treatment have led to problems such as strained medical resources in particular, and most hospitals are overcrowded. The rapid development of Internet healthcare has slowed down this situation to a large extent, giving people more channels and methods to learn about health information and related services.Well-known online health communities in China, such as Weimed,Ping An Good Doctor and Ali Health, are increasingly used as platforms for communication and information sharing between physicians and patients.Patient satisfaction has become an important indicator for evaluating the effectiveness of treatment, and it can provide a reference for improving the quality of medical services and optimizing the medical service process.Patient satisfaction with online health communities may influence patients’ willingness to continue to search for health information through the Internet in the future and how often they use it,thus affecting their accumulation of health information and health literacy in their daily lives, and ultimately influencing their health-related behavioral decisions, including their compliance with medical prescriptions and medications.Patient compliance refers to how patients comply with their primary care physician’s diagnosis and recommended treatment plan during offline visits, and is particularly important for improving cure rates. However, most of the studies on patient satisfaction in China focus on offline clinics and are mainly obtained through questionnaires or interviews.Few scholars have used natural language processing techniques such as topic recognition to conduct satisfaction studies on the evaluation content of the online health domain. In addition, the current domestic research on patient adherence focuses on care services and medication use for patients with chronic diseases, while research in informatics and behavior is still to be developed.Based on the online health community in China, this study uses LDA topic identification technology to extract and summarize the satisfaction topic dimensions of patient evaluation data in the community;on this basis,it combines self-determination theory, use and satisfaction theory, social information processing theory, and information asymmetry theory to propose an empirical relationship between patient satisfaction and its associated variables in the online health community on patient compliance during offline visits. The research model was developed by selecting latent variables such as perceived physician quality, perceived online health community quality, patient satisfaction, health information seeking behavior, perceived information asymmetry,e-health literacy, and patient adherence, and conducting hypothesis testing and control variable effects analysis through structural equation modeling with reference to established scales from previous scholarly studies.Based on the experimental results,we provide scientific explanations for patients’ health decisions and suggestions for the improvement of patient satisfaction, patient e-health literacy, and patient compliance. The results of the study found that, first, when examining the thematic focus dimensions of patient satisfaction in the online health community,it was found that patient satisfaction was focused on the service attitude, medical skills, and communication level of the online consulting physicians,as well as on the degree of standardization and ease of use of the online health community. However, the online health community is a basic carrier for patients to consult and search for information online, and patients’ feelings of using it will implicitly influence patients’ evaluation.Second,in studying the effect of online health community patient satisfaction and its associated variables on patient adherence,it was found that patient satisfaction is positively influenced by patients’ perceptions of the quality of online consulting physicians and the quality of online health community, and further influences their adherence to medical advice and medication during their actual offline visits by acting on their health information search behavior and e-health literacy, etc. In addition, the effect of age, gender, and education level on the adherence to medical advice and medication in the offline consultation process. In addition, age, gender, education level, and residence type did not have significant effects on the control variables of the structural equation model, except for residence type, which had a significant effect on patients’ e-health literacy.Combining the results of the theme mining and empirical research experiments, this paper helps physicians understand the doctor-patient relationship in the Internet health environment and suggests references for online health community managers regarding future development. |
