The Application Of Modified Dahuang Mudan Decoction In The Treatment Of Acute Peritonitis

Objective: In general surgery,acute peritonitis is the most common acute abdomen.Therefore,the aims of this study is exploring the clinical value of Modified Dahuang Mudan Decoction in the treatment of acute peritonitis,and trying to provide new ideas for the treatment of different types of diseases in abdominal infection.Methods: We retrospectively analyzed the clinical data of 143 patients with acute peritonitis,who were hospitalized in the Department of General Surgery,Shaoxing Hospital,China Medical University from December 2018 to December 2020.All patients in the control group used traditional symptomatic treatment methods,such as anti-infection,fluid rehydration,nutritional support,laparoscopic surgery and gastrointestinal decompression,etc.All patients in the observation group(integrated traditional Chinese and western medicine treatment group)were added Modified Dahuang Mudan Decoction to treat acute peritonitis.The ingredients of the prescription were: rhubarb 12 g,peony bark 3g,winter melon kernel 30 g,peach kernel9 g,glauber’s salt 9g,patronia 15 g,dandelion 15 g.The rest of the treatment plan was the same as the control group.Through the research and comparison of the hospitalization time,the intestinal function recovery time,the body temperature recover normal time,white blood cell count,neutrophil count,high-sensitivity C-reactive protein,procalcitonin and other indicators on the 5th postoperative day,analysis the role of Modified Dahuang Mudan Decoction in the treatment of acute peritonitis played.Results: 1.Among the 143 patients,there were 82 cases in the observation group,including 41 males and 41 females,aged(43.57±13.80)years old,including 19 cases of gastric perforation,14 cases of duodenal perforation,18 cases of acute simple appendicitis and 31 cases of acute purulent appendicitis,;61 cases in the control group,including 31 males and 30 females,aged(42.84±14.13)years old,including 10 cases of gastric perforation,12 cases of duodenal perforation,16 cases of acute simple appendicitis and 23 cases of acute purulent appendicitis.2.Among the observation group,the hospitalization time was(6.83±2.42)days,the postoperative intestinal function recovery time was(1.63±0.62)days,and the body temperature recover normal time was(2.51±1.03)days;among the control group,the hospitalization time was(8.10±2.61)days,the postoperative intestinal function recovery time was(2.21±0.41)days,and the body temperature recover normal time was(4.10±1.59) days.3.Among the observation group,the white blood cell count was(5.68±1.44)*10^9/L on the 5th postoperative day,and the neutrophil count was(3.65±1.28)*10^9/L on the 5th postoperative day,the value of CRP was(8.20±6.62)mg/m L on the 5th postoperative day,the value of PCT was(0.41±0.25)ng/m L on the5 th postoperative day;among the control group,the white blood cell count was(6.42±1.49)*10^9/L on the 5th postoperative day,and the neutrophil count is(4.10±1.25)*10^9/L on the 5th postoperative day,and the value of CRP is(16.76±9.81)mg/m L on the 5th postoperative day,the value of PCT was(0.95±0.58)ng/m L on the 5th postoperative day.4.Comparison of various clinical indicators: It is no statistical difference between the observation group and the control group in general conditions such as age and gender,and the severity of the disease is comparable.The observation group’s various observation index values are all better than the control group.Among them,there were statistically significant differences in all indexes such as hospitalization time,the postoperative intestinal function recovery time,white blood cell count,neutrophil count,the values of CRP and PCT on the 5th postoperative day(all P<0.05).Conclusion: in this study,the Modified Dahuang Mudan Decoction showed a significant effect in the treatment of secondary peritonitis.Compare with the control group,the hospitalization time and postoperative intestinal function recovery time of the observation group were significantly shorten,and the inflammation subsided faster.The observation group’s inflammatory indexes on the 5th postoperative day such as white blood cell count,neutrophil count,CRP and PCT values were all lower than those in the control group.Adjuvant treatment of patients with acute peritonitis with Modified Dahuang Mudan Decoction can significantly shorten the hospitalization time,reduce the level of inflammatory factors,and improve the clinical efficacy.
