Objectives: By comparing the clinical features of the second branchial cyst and the large cystic lymphangioma of the neck, how to differentiate the two diseases was discussed. Methods: The clinical data of the inpatients who were pathologically diagnosed as second branchial cyst and large cystic lymphangioma of neck in our department were retrospectively analyzed, including 112 cases of second branchial cyst and 30 cases of large cystic lymphangioma of neck.SPSS23.0 software was used for statistical analysis of the obtained data.Mean analysis and two independent samples t test were performed on the measurement data P<0.05 is statistically significant.Chi-square test was planned for the count data, because 30 cases of large cystic lymphangioma of the neck in this group,n<40.Therefore,Fisher’s exact probability method was used to test, and P<0.05 is statistically significant. Results: The mean age of patients with second branchial cyst was 27.39±14.17 years, and the mean age of patients with large cystic lymphangioma of the neck was17.29±17.17 years, and the difference was statistically significant(P<0.05).In ultrasound images, the second branchial cleft cyst had a clear boundary in 94% of cases, and the second branchial cleft cyst had a clear boundary in 6% of cases, and the neck large cystic lymphangioma had a clear boundary in 43.75% of cases, and the second branchial cleft lymphangioma had a clear boundary in 56.25% of cases. The difference in the degree of clear boundary between the two groups was statistically significant(P<0.05);84% of the second branchial cyst had echoes,16% of the second branchial cyst had echoes, and 50.00% of the neck large cystic lymphangioma had echoes and 16% of the neck cystic lymphangioma had echoes. The echo difference between the two groups was statistically significant(P<0.05).In MRI images, the regular shape of the second branchial cleft cyst accounted for 82.35%,the irregular shape accounted for 17.65%,the regular shape of the neck large cystic lymphangioma accounted for 35.29%,and the irregular shape accounted for 64.71%,the difference of shape between the two was statistically significant(P<0.05);In MRI images of the second branchial cleft cyst, there was no separation, and 47.06% of the patients had separation in the neck large cystic lymphangioma, and 52.94% of the patients had no separation. The difference of separation between the two was statistically significant(P<0.05).16 patients with second branchial cleft cyst underwent both ultrasound and MRI examination, and 18.75% of the patients were confirmed by ultrasound and62.50% by MRI.The difference in the diagnosis rate between the two examinations was statistically significant(P<0.05).9 patients with large cystic lymphangioma of the neck underwent ultrasound and MRI examination at the same time.About 22.22%of the patients were diagnosed by ultrasound, while 88.89% of the patients were diagnosed by MRI.There was a statistically significant difference in the diagnosis rate between the two examinations(P<0.05).
