| BackgroudOsteoporosis(OP)is a systemic metabolic disease characterized by bone mass loss and abnormal microstructure of bone tissue.It usually increases bone fragility and easily leads to fracture.Calcium regulating hormone is closely related to the occurrence of OP.At present,calcitonin(CT)has been widely used in the clinical treatment of OP.Domestic and foreign studies have shown that single application of salmon calcitonin nasal spray or calcitriol in the treatment of OP patients has a certain curative effect,but in postmenopausal women with osteoporosis,the combination of the two drugs and the observation of the changes of serum bone metabolism markers are rare.Therefore,we selected bone mineral density(BMD)and bone-specific alkaline phosphatas(BALP),bone glaprotein(BGP),type I procollagen C-terminal peptide(CTX-Ⅰ),tartrate-resistant acid phosphatase 5b(TRACP-5b)and other bone turnover metabolic indicators in this study to observe the clinical effect of salmon calcitonin combined with calcitriol in the treatment of OP,in order to provide help for clinical precise treatment of OP.ObjectiveThe changes of salmon calcitonin combined with osteoporosis on bone density(BMD)and bone conversion index concentration in postmenoporal female osteoporosis patients after menopause were analysed to explore the protective effect of osteoporosis in postmenoporal women.Methods1.Objectives: 70 postmenopausal women were selected for treatment at the central hospital of Luohe,in accordance with the expert consensus on diagnostic criteria for osteoporosis(2014 edition).70 cases were randomly divided into two groups: 35 cases in the observation group,the application of nasal spray salmon calcitonin sprayed one time a day,while oral osteosterol softgels,oral one capsule in the morning;and the other 35 cases in the control group,the application of the same measurement as the observation group of osteosterol softgels at the same time oral calcium,dose of two capsules/time,prebedtime oral.all cases were followed up for 12 months.2.Detection method: blood samplesfrom all study subjects were collected before and after 12 months of treatment Dual energy X-ray absorption(DXA)device was used to measure the bone density of the hip,lumbar spine,greater trochanter of femur,and neck of femur.Serum concentrations of BALP,BGP,CTX and TRACP-5b were determined by ELISA.3.Statistical processing: SPSS20.0 software was used to process data.When all measurement indicators conform to normal distribution,they are showed by mean ±standard deviation(x s).Chi square test was used for counting data,the one-Way ANOVA variance analysis was used for intergroup comparison,and the T-test was used to compare the mean of two groups.P<0.05 was a statistically significant difference.Results1.Changes of BMD in the group before and after treatment: compared with before treatment,BMD of the skeletal part,lumbar spine,femoral neck and greater trochanter in the observation group increased after treatment(P<0.05);BMD of the mandibular skeletal part in the control group increased after treatment(P<0.05),but BMD of lumbar spine,femoral neck and greater trochanter had no significant difference after treatment.2.Changes of BMD between two groups after treatment: compared with the control group,the increase of BMD of greater trochanter and lumbar vertebrae in the observation group was better than that in the control group(P<0.01),and there was no significant difference in BMD of bone and femoral neck between two groups.3.Comparison of the concentration changes of bone turnover metabolic indicators before and after treatment : compared with before treatment,the bone formation indicators(BALP and BGP)of the observation group after treatment significantly increased(P<0.05),and the levels of bone resorption indicators(TRACP-5b and CTX-Ⅰ)significantly decreased(P<0.05);while the levels of bone turnover metabolic indicators(BALP,BGP,TRACP-5b,CTX-Ⅰ)of the control group had no significant difference before and after treatment.4.Comparison of the concentration changes of bone turnover metabolic indexes between the two groups: before treatment,there was no significant difference in bone metabolism indexes(BALP,BGP,TRACP-5b,CTX)between the two groups;After treatment,the bone formation indexes(BALP and BGP)in the observation group were higher than those in the control group;The levels of tracp-5b and CTX-Ⅰ decreased in the observation group(P<0.01).5.Comparison of therapeutic effect: the total effective rate of the observation group was higher than that of the control group(P<0.05).Conclusions Salmon calcitonin combined with calcitriol can significantly inhibit bone resorption,reduce bone turnover rate,and improve bone mineral density of lumbar spine,femoral neck and greater trochanter in the treatment of postmenopausal women.Its curative effect is better than calcium combined with calcitriol. |
