| Objective:Objective to understand and master the epidemiological characteristics of iatrogenic wounds and provide strategies for clinical prevention and treatment of iatrogenic wounds by retrospective analysis of the clinical data of patients with iatrogenic wounds admitted to Jiangxi burn center(burn and wound repair center,First Affiliated Hospital of Nanchang University)in recent ten years.Methods:Collection on January 1,2011 to December 31,2020 patients with iatrogenic wounds were burn center in jiangxi province with the clinical data,statistics the total,the urban and rural distribution,gender,age,wound causes,source department,parts distribution,area and degree,hospitalized time,treatment and outcome,such as data,and the iatrogenic and in patients with iatrogenic wound surgery number,length of hospital stay and cost comparison,contrast and iatrogenic wound before and after the change of the quantity,for five years for epidemiological analysis.Results:1.A total of 156 iatrogenic wound patients were admitted to Jiangxi burn injury center from 2011 to 2020,accounting for 8%of the total number of wounds.among which 77(49.4%)were male iatrogenic wounds and 79(50.6%)were female iatrogenic wounds,male:female=0.97:1.55 cases(35.2%)of iatrogenic wounds occurred in young people(18-44 years old),accounting for the largest proportion.Secondly,46 cases(29.6%)had iatrogenic wounds in the elderly segment(60-89years old).There were 49 iatrogenic wounds in urban patients(31.4%),and 107iatrogenic wounds in rural patients(68.6%).Rural patients:urban patients=2.18.1.2.By subject department,96 cases(61.6%)were found in surgery department,including 52 cases in general surgery department,23 cases in orthopedics department,8 cases in burn orthopedics department,9 cases in brain surgery department,2 cases in cardiothoracic surgery department,and 2 cases in urology department.Secondly,22 cases of obstetrics and gynecology and 3 cases of pediatrics,accounting for 16%;There were 22 cases(14.1%)in internal medicine department,including 15 cases in oncology department,6 cases in traditional Chinese medicine department and 1 case in nephrology department.And 13 cases(8.3%)due to physical therapy in health care Settings.According to the etiology,there were 103 cases of unhealed wound(66%),35 cases of iatrogenic burn(22.4%),14 cases of radioactive ulcer(9%),and 3 cases of drug extravasation and 1 case of iatrogenic pressure ulcer.3.According to the injury site,67 cases(42.9%)were in the trunk,accounting for the largest proportion,followed by 54 cases(34.6%)in the lower limbs,and less in the upper limbs.According to the time of admission,109 cases(69.9%)were admitted to the hospital after the wound was larger than 1 month without healing,and47 cases(30.1%)were admitted after the wound was smaller than 1 month without healing.4.Iatrogenic wounds(except iatrogenic burns)were mostly severe,with an average area of(6.2±2.1)*(4.6±1.7)cm~2.The majority of iatrogenic burn patients were moderate burn,accounting for 68.6%.There were only 2 cases of severe burns.The minimum burn area was 1%TBSA,the maximum was 28%TBSA,and the average burn area was 5.5%TBSA.During the hospitalization of iatrogenic wound patients,121 cases(77.6%)were treated with surgery,and the number of operations was 1.62±1.53 times.The surgical methods were debridement,negative pressure sealing drainage,skin grafting,skin flap transfer,etc.,and 35 cases(22.4%)were treated with conservative dressing change.126 patients were cured and discharged,and 12 patients were discharged automatically without recovery,with a cure rate of87.5%(the number of patients without recovery and discharged automatically was excluded here).0 cases died.The length of hospitalization was 25.62±21.32 days,and the hospitalization cost was 42544.44±1114.80 yuan.5.There were significant differences between iatrogenic and non iatrogenic wounds in operation times,hospitalization time and cost(P<0.05);The incidence of iatrogenic wounds was on the rise in the first five years,and the most obvious increase was the number of postoperative wounds,followed by iatrogenic burns(P<0.05).Conclusion:The number of iatrogenic wounds in our center is increasing,mainly due to surgery and iatrogenic burns.The second treatment increased the psychological and economic burden of patients.In order to reduce the incidence of iatrogenic wounds,we should pay attention to and strengthen medical safety,and actively prevent the occurrence of iatrogenic wounds. |
