Aims:Diabetes has become one of the most common chronic disease.2-h PG rises sharply with age.So we explore the glucose metabolism status and risk factors of elevated 2-h PG in rural areas in northern China,and provide theoretical basis for the prevention and management of diabetes in rural areas.Materials and methods:A questionnaire was designed in advance,including three parts: demographic characteristics and information of lifestyle and medical history.Demographic characteristics include name,gender,date of birth,age,years of education,average income;lifestyle information includes smoking,drinking,physical exercise,sleep time,whether or not snoring and second-hand smoking;medical history includes hypertension,diabetes,hyperlipidemia and stroke.After standard training,researchers used questionnaires to survey Tianjin rural populations in a face-to-face manner,collecting demographic characteristics,lifestyle information and medical history.Doctors measured the height,weight,WC,SBP,DBP and heart rate for the participants and collected fasting venous blood.The subjects then took a standard 75 g glucose solution,and venous blood was taken again 2 hours later.All the measurements,including TC,TG,HDL-C,LDL-C,FPG,2-h PG,hs-CRP,SUA,blood routine,ALT,AST and TBIL,are performed at Guangzhou King Med Diagnostics Group Co.,Ltd..Statistical analysis was performed to explore the glucose metabolism status and risk factors of elevated 2-h PG about middle-aged and elderly in rural Tianjin.Results:This cross-sectional study recruited 3,077 rural residents aged 50 and over.The quartile range of the 2-h PG was <5.62mmol/L,5.62 ~ 6.83mmol/L,6.84 ~8.57mmol/L and ≥8.585 mmol/L.The prevalence of i-IFG,i-IGT,IFG + IGT and DM among middle-aged and elderly people in rural areas of Tianjin was 3.6%,16.1%,3.6% and 24.6%.The risk factors of elevated 2-h PG were women(OR,1.277;95%CI,1.002-1.627),older age,drinking(OR,1.343;95% CI,1.050-1.719),physicalexercise(OR,1.208;95% CI,1.035-1.409),snoring(OR,1.212;95% CI,1.034-1.420),hypertension(OR,1.215;95% CI,1.006-1.167),obesity(OR,1.441;95% CI,1.156-1.793),high levels of heart rate(OR,1.014;95% CI,1.007-1.020),TG(OR,1.328;95% CI,1.221-1.445),WBC(OR,1.105;95% CI,1.051-1.163),hs-CRP(OR,1.037;95% CI,1.017-1.058),SUA(OR,1.001;95% CI,1.000-1.002),and ALT(OR,1.009;95% CI,1.003-1.016).Smoking(OR,0.555;95% CI,0.426-0.732)was oppositely associated with 2-h PG.Conclusions:The prevalence of prediabetes and diabetes are significantly high in rural areas of Tianjin.Cardio-cerebral vascular events,renal failure,diabetic retinopathy,diabetic foot,and even amputations may occur in the long-term,seriously threatening the health of rural residents.Therefore,for risk factors that can be controlled,such as hypertension,obesity,hyperlipidemia,hyperuricemia,etc.,health education should be strengthened to actively control BP and plasma glucose to reduce the incidence of prediabetes and diabetes.Standardized blood sugar management,lifestyle interventions and drug treatments should be given to patients with diabetes to avoid the complications,reduce medical burden,and improve quality of life in future.
