ObjectiveTo understand the distribution,main clinical manifestations and imaging features of common benign breast diseases.To investigate nonstandard medical treatment behavior and nonstandard treatment rate of patients with benign breast disease.And the relationship between the outcome of disease and the risk factors such as bad habits and nonstandard medical treatment.MethodsIn this study,patients with benign breast diseases and breast cancer patients with benign breast disease history received in the breast surgery outpatient department of the fourth central hospital of tianjin during the period of 2019.1-2019.12 were selected by random sampling and conducted a cross-sectional study,and relevant information of the patients was recorded through questionnaires.The questionnaire was collected in the form of interview.Then,the collected paper questionnaires were summarized,and the data were input by Excel.The data were further simplified and classified,and all valid data were statistically analyzed by SPSS 20.One-way analysis of variance was used for continuous numerical variables,chi-square test was used for bidirectional disordered classification variables,and Logistic regression analysis was used for the influence of multiple classification variables on dependent variables.Results1.All the 478 patients in the diagnosis of mammary gland hyperplasia 378(79.1%)cases,breast fibroadenoma 52(10.9%)cases,and other benign breast diseases 48(10.0%)cases.The overall average age was 44.42±12.80 years old.2.The chief complaints of 378 cases of breast hyperplasia were divided into four categories:(1)193 cases of breast pain;(2)42 cases of nodules or masses found in physical examination or breast self-examination;(3)130 cases were previously diagnosed as mammary hyperplasia and came here for a review,among them,118 cases were diagnosed as mammary hyperplasia with breast nodules;(4)13 cases of nipple discharge.In addition,there were significant differences in the age distributionof patients with the four main complaints of breast hyperplasia.The ultrasonic examination results were divided into three categories:(1)no abnormal 74 cases;(2)mammary gland thickened and echo thickened in 85 cases;(3)Breast nodules or masses 219 cases.Ipsilateral axillary lymph nodes: positive 22 cases;negative 356 cases.3.In 478 patients,286 were found to be nodular lesions of the breast by ultrasound examination,and the main manifestations were the external upper quadrant of the breast,and small nodules less than 1cm in BI-RADS 3.In addition,there were significant differences in age distribution of patients with breast nodules in different categories.4.There were significant differences in BMI distribution among patients with different menstrual conditions,no significant differences between the normal and abnormal menstrual groups,but they were significantly lower than the menopausal group.5.Among the 478 patients,210 had irregular medical behavior,and the irregular outpatient rate was 44%.Logistic regression analysis found that disease awareness,age,family income and health care type were influencing factors of patients’ medical treatment behavior.6.The results showed that there were significant differences in the effects of disease fear,disease awareness,medical treatment attitude and follow-up frequency on disease outcome.Logistic regression analysis found that these four indicators were all risk factors affecting the outcome of benign breast disease.7.Among the 63 patients with benign breast tumors,17 had a history of dependence on proprietary Chinese medicine.8.Among the 41 breast cancer patients with benign breast disease history,13 patients were followed up normally,and 28 patients were not followed up and did not seek medical treatment actively.There were significant differences in tumor nature,tumor size,lymph node metastasis and clinical stage in the two groups.Conclusions1.The benign breast diseases diagnosed in the outpatient department of breast surgery were mainly breast hyperplasia and breast fibroadenoma.The two diseasesaccounted for about 90% of the benign breast diseases in the outpatient department and the peak age of incidence was 20-50 years old.2.BMI may be a risk factor affecting the incidence of benign breast disease in postmenopausal women.3.About 44% of patients with benign breast diseases in the breast surgery outpatient department have non-standard medical treatment behavior,and the probability of non-standard medical treatment is significantly increased for patients who do not understand the disease,are 40-50 years old,have an average annual income of less than 20,000 yuan in their families,and have medical insurance type of residents health care.4.Non-standard medical treatment,have no understanding of the disease and fear of the disease significantly increase the risk of aggravation.5.Patients with irregular follow-up are more likely to develop invasive breast cancer,with larger tumor volume,greater risk of lymph node metastasis,more severe clinical staging,poorer prognosis,and higher medical costs.6.There were misdiagnosis and overuse of proprietary Chinese medicine in benign breast tumors in outpatient department.
