Application Study Of Oral Contrast-enhanced Ultasonography In The Diagnosis Of Gastric Space-occupying Diseases

Objective To analyze the characteristics of different oral CEUS images and to study the clinical value of CEUS in the diagnosis of common gastric diseases.Materials and Methods From March 2012 to April 2019,106 patients with clinical symptoms such as nausea,heartburn,acid reflux,and upper abdominal discomfort were admitted to our hospital for general two-dimensional ultrasound and CEUS,and were compared with the results of electronic gastroscopy and pathology.Results The detection rates of gastric cancer,gastric stromal tumor,gastric mucosa polyp,gastric ulcer and gastritis by OCUS were 87.5%(49/56),100%(2/2),71.4%(5/7),65.2%(15/23)and 42.8%(9/21),respectively.The diagnostic coincidence rates of OCUS and gastroscopy for gastric cancer,ulcer,polyp and benign mucosal diseases were 94.2%,68.1% and 55.1%,respectively.Compared with the surgical pathological diagnosis results,the diagnostic coincidence rate(accuracy rate)of ORAL gastrointestinal OCUS in TNM staging was 60.0% in Stage T1,71.4% in Stage T2,76.0% in stage T3,and 83.3% in stage T4,respectively.The total T staging accuracy rate was 75.0%,as shown in Table 1.The diagnosis coincidence rate of oral gastrointestinal contrast-enhanced ultrasonography on stage N: 84.2%,70.5%,72.7%,77.8%,N0,70.5%,N2,and N3.The thickness of gastric wall was significantly different between benign and malignant gastric diseases(T =3.283,P < 0.05).When the gastric wall thickness was 7.854,the ROC curve obtained the maximum area under the curve of 0.883,which had the highest sensitivity for differentiating benign from malignant lesions.Conclusions Through analysis,the features of ce US images of gastric window are obvious and have special and different characteristics in the diagnosis of common gastric diseases.It has practical value in the detection of gastric cancer and common benign diseases of the stomach,and has clinical significance in the clinical staging and preoperative evaluation of gastric cancer.Moreover,it is of great significance in outpatient general survey and disease screening due to its relatively simple operation,small pain degree of patients and easy acceptance.
