Objective:To compare the clinical efficacy of minimally invasive percutaneous plate osteosynthesis(MIPPO)and open reduction and internal fixation(ORIF)in the treatment of surgical neck fractures of the humerus,and to provide reference for clinical treatment of surgical neck fractures of the humerus.Methods:According to the inclusion and exclusion criteria of this study,60 patients with surgical neck fractures of the humerus admitted to the orthopedic ward of the Affiliated Hospital of Shandong University of Traditional Chinese Medicine from October 2018 to January 2020 were randomly divided into 32 cases in the percutaneous minimally invasive plate internal fixation group(MIPPO group)and28 cases in the traditional open reduction and internal fixation group(ORIF group).Both groups were given Chinese medicine adjuvant treatment.The operation time,intraoperative blood loss,postoperative VAS pain score,fracture healing time,hospital stay and postoperative complications were compared between the two groups.According to the Neer shoulder joint function score,the shoulder joint function was evaluated at 3 months after operation,6 months after operation and at the last follow-up.Results:The 60 patients included in this study were all followed up for 12-18 months,with an average follow-up time of 14.7 months.There was no statistically significant difference in general information between the two groups of patients(P>0.05),and they were comparable.Comparison of intraoperative observation indexes between the two groups showed that the operation time,the intraoperative blood loss in the MIPPO group were lower than those in the ORIF group,and the difference was statistically significant(P<0.05).Comparison of postoperative observation indexes between the two groups,the MIPPO group was better than the ORIF group in terms of fracture healing time,VAS pain score on the first day after surgery,VAS pain score on the third day after surgery,and VAS pain score on the fifth day after surgery.There is statistical significance(P<0.05).At 3 months after operation,the Neer shoulder function score in the MIPPO group was better than that in the ORIF group(P<0.05),but there was no statistically significant difference in the function score between the two groups at 6 months after surgery and at the last follow-up(P>0.05).There was no significant difference in hospital stay and postoperative complications between the two groups(P>0.05).Conclusion:In this study,compared with the traditional open reduction and internal fixation(ORIF),the minimally invasive percutaneous plate internal fixation(MIPPO)has the advantages of less intraoperative blood loss,shorter operation time,less postoperative pain,shorter fracture healing time,and quicker recovery of shoulder joint function.
