Objective: To explore the expression levels of serum complement factor B(CFB)and complement factor D(CFD)in patients with type 2diabetes mellitus(T2DM)complicated with peripheral neuropathy,and preliminarily explore their correlation in the pathogenesis of diabetic peripheral neuropathy(DPN).Methods:110 patients who were hospitalized in the Department of Endocrinology,Affiliated Hospital of Jining Medical College from October 1,2019 to October 1,2020,who met the inclusion criteria for T2 DM were selected,they were divided into diabetic peripheral neuropathy group(DPN group,n = 60)and simple type 2 diabetes mellitus group(T2DM,n = 50)according to whether they were complicated with diabetic peripheral neuropathy;In addition,52 healthy people who were examined at the Health Examination Center of the Affiliated Hospital of Jining Medical College during the same period and met the inclusion criteria were selected as the normal control group(NC group,n=52).The subjects’ age,sex,duration of diabetes,height,weight,blood pressure,ALT,AST,SCR,BUN,TC,TG,FPG,Hb A1 c,FT3.,FT4,TSH and other indexes were collected.The expression levels of CFB,CFD and Tumor necrosis factor-α(TNF-α)in serum of each group were determined by enzym-linked immunosorbent assay(ELISA),and the correlation between CFB,CFD and various clinical indicators was analyzed.Logistic regression was used to analyze the influencing factors of DPN.Results:One-way analysis of variance showed that the levels of CFB and CFD in the DPN group were higher than those in the T2 DM group and NC group[CFB(845.43±101.10)vs(792.19±116.59)vs(739.20±123.43),P<0.05]、[CFD(491.71±41.03)vs(467.58±45.16)vs(445.16±50.47),P<0.05].Pearson correlation analysis showed that the expression level of CFB was positively correlated with Hb A1 c,FPG and TNF-α(P <0.05).The expression level of CFB was negatively correlated with FT3、DBIL and TBIL(P<0.05).The expression level of CFD was positively correlated with SBP,Hb A1 c,FPG and TNF-α(P<0.05).The expression level of CFD was negatively correlated with FT3DBIL、and TBIL(P<0.05).Logistic regression analysis showed that after excluding SBP、HbA1c、FPG、FT3、DBIL、TBIL、TNF-α and other confounding factors,CFB and CFD were still the influencing factors for the occurrence and development of DPN(P<0.05).Conclusion:1.The expression levels of CFB and CFD in the peripheral blood of patients with diabetic peripheral neuropathy are significantly increased,suggesting that CFB and CFD are involved in the occurrence and development of DPN.2.The expression level of TNF-α in the peripheral blood of patients with diabetic peripheral neuropathy is significantly increased,confirming that TNF-α promotes the occurrence and development of DPN.
