The Value Of BIASP Combined With PLR And NLR In Evaluating The Prognosis Of Acute Pancreatitis

Objective: Explore the relationship between BISAP score,neutrophil/lymphocyte ratio(NLR),platelet/lymphocyte ratio(PLR)and the severity of AP,and further explore the early predictive value of BISAP score combined with NLR and PLR on the severity of AP.Methods: A retrospective collection of AP patients admitted to each department of Yijishan Hospital of Wannan Medical College from September 2019 to September 2020.The patients were screened according to the AP classification standards revised in Atlanta in 2012.They were divided into 3 groups according to their condition: MAP group,MSAP group and SAP group.Within 24 hours of the patient’s admission,the patient’s peripheral venous blood was drawn for blood routine and biochemical examinations,and abdominal CT imaging was performed,and then the BISAP score,NLR and PLR values were calculated,relevant data were recorded in detail,and the differences between the 3 groups were statistically analyzed.Make receiver operating characteristic curve,calculate area under the curve,calculate the best Youden index,sensitivity,specificity,etc.,Further explore the predictive value of BISAP score,NLR,PLR,and PLR to the severity of SAP.Results: A total of 151 AP patients were enrolled,including 787 in the MAP group,24 in the MSAP group,and 49 in the SAP group;71 males and 80 females;They are between 18 and 94 years old.Etiology classification: 65 cases of biliary origin,27 cases of hyperlipidemia,20 cases of alcohol,23 cases of overeating,16 cases of unknown cause.There was no statistical difference in the basic data of the three groups of patients(height,weight,age,gender).BISAP score of MAP group,MSAP group and SAP group(0.70±0.64,1.20±0.89,1.61±0.91),NLR(4.07±4.17,12.79±7.27,17.31±20.67),PLR(140.91±86.7,217.96±91.72,242.79 ±164.71)all increased sequentially,the difference was statistically significant.NLR,PLR,BISAP scores are all positively correlated with the severity of the patient’s condition(r=0.64,r=0.31,r=0.65).In terms of disease prediction,the area under the ROC curve(AUC)of BISAP score,NLR,PLR,BISAP +NLR+PLR predicting AP disease is 0.86(95% CI: 0.77～0.90)and 0.83(95% CI: 0.76～0.81),0.64(95% CI: 0.56～0.75),0.90(95% CI: 0.85～0.95,P<0.001).Their sensitivity and specificity are(0.80,0.89)(0.82,0.74)(0.57,0.75)(0.84,0.88),respectively.It is found that BISAP + NLR + PLR has a higher AUC value and a higher sensitivity and Specificity,it has a higher value for predicting AP.Conclusion: BISAP score,NLR,and PLR are all positively correlated with the severity of AP disease.BISAP score has the strongest correlation.BISAP+NLR+PLR has a higher value for early predicting the severity of AP patients.
