OBJECTIVE: To investigate the blood glucose fluctuations of type 2diabetes patients with metformin alone who have poor glucose-lowering effect after combined use of acarbose or repaglinide.Methods: From January 2018 to July 2019,patients with type 2 diabetes in the First Affiliated Hospital of Xinjiang Medical University were collected.According to the inclusion and exclusion criteria,it was divided into two groups:136 patients in the metformin + acarbose group(M+A)and 137 in the metformin+repaglinide group(M+R).With the exception of metformin,all patients took acarbose 100 mg or repaglinide 2 mg three times a day until the end of follow-up.Collect patients’ general data informations,examination indicators,7 fingertip blood glucoses and calculate SDBG,PPGE,LAGE,MAGE.Comparing the fluctuations of blood glucose stability in M+A group and M+R group in type 2 diabetes patients who failed to control metformin alone by 1 week and 12 weeks after treatment.Results:(1)There were no significant differences(P> 0.05)between the general data(gender,age,ethnicity,BMI,Hb A1 c,lipids,etc.)of M + A and M + R in this study;MAGE,SDBG,PPGE,LAGE and other blood glucose stability indicators also have different degrees of reduction with significant statistical significance.(2)Both groups can significantly reduce Hb A1 c and GSP;and the index of M+A group decreases more significantly,suggesting M+A may be better.(3)FPG,Hb A1 c and Hb A1 c were positively correlated with MAGE(r in group M+A was 0.03606,0.5799 and 0.07843,P was 0.6769,< 0.0001 and 0.3641 respectively.In M+R group,r was 0.04458,0.5568,and 0.09385,and P was 0.6049,<0.0001,and 0.2754,respectively.)(4)The incidence of hypoglycemia in the two groups of patients during the follow-up process is low,indicating high safety.Conclusion: Acarbose and repaglinide can significantly reduce the FPG and 2h PG,and can also reduce the blood glucose fluctuation indexs(MAGE,SDBG,PPGE,LAGE).The M + A group is better.
