Abstract/Summary:
Diabetic nephropathy is a common complication of diabetic patients caused by different etiology and pathogenesis.It ranks first in end-stage renal disease.It is also a global end-stage renal disease and the main cause of diabetic renal failure.So far,the pathogenesis of diabetic nephropathy is not clear,and little is known about the early diagnosis of diabetic nephropathy.Because of the social development and the change of people’s living structure,the incidence rate of diabetes is on the rise.The incidence of diabetic nephropathy is increasing year by year.Therefore,the early diagnosis and treatment of diabetic nephropathy is of great clinical significance to delay the deterioration of the disease.Whether we can find a sensitive laboratory test index,not only has an important value for the early diagnosis of diabetic nephropathy,but also provides a basis for the clinical diagnosis of diabetic nephropathy.Through the biochemical indicators of the selected population,the changes of various indicators in patients and the clinical correlation between the progression of diabetic nephropathy(DN)were analyzed,and the clinical significance of serum CysC and BUN in the diagnosis of early diabetic nephropathy was discussed in detail.The retrospective analysis method was used to randomly select the patients who were hospitalized in the Department of Nephrology,the Fifth Affiliated Hospital of Xinjiang Medical University from March 2017 to October 2018 as the research objects.According to the relevant standards,the cases were collected,sorted out and excluded.Finally,168 samples were selected and divided into 51 cases of diabetes group(DM group)and 117 cases of early diabetic nephropathy group(DN group),and 57 cases of physical examination were selected They were compared as control group(NC group).Collect and calculate relevant data of selected samples,including gender、age、height、weight、body mass index(BMI)、arterial blood pressure(SNP、DNP)、triglyceride(TG)、total cholesterol(TC)、high-density lipoprotein(HDL-C)、low-density lipoprotein(LDL-C)、serum creatinine(CR)、urea nitrogen(BUN)、Cystatin C(Cys C)、UA、glucose、 mAlb、24h UAE、α 1-MG、β 2-MG、sRBP.Spss21.0 was used for statistical analysis,and Shapiro Wilk method was used to test the normal distribution.The mean ± standard deviation is used to represent the parameters of normal distribution,and the differences between groups are compared with the clinical test indexes and renal function test indexes of each test group by means of variance analysis;the Pearson correlation coefficient is used to measure the correlation between two independent samples,if p If it is less than 0.05,there is asignificant difference between the two independent samples;the risk factors of the indicators in the diagnosis of DN are analyzed by multiple linear regression analysis;the value of each indicator and its joint analysis in the diagnosis of early diabetic nephropathy is evaluated by receiver operating characteristic curve(ROC curve).The general data and biochemical indexes of NC,DM and DN groups showed that there was no significant difference in sex、 age、height、weight、BMI、SNP、 DBP、TG、TC、HDL-C and LDL-C among the three groups(P > 0.05),Cr and BUN among the patients(P > 0.05)The levels of Cr、BUN、CysC、UA、mAlb、α 1-MG、 β 2-MG and srbp in DM group were significantly higher than those in NC group(P < 0.05).The results of linear regression and ROC curve analysis showed that the indexes of CysC and BUN were closely related to the injury degree of diabetic nephropathy.It is found that the early diagnosis of CysC and BUN diabetic nephropathy has high clinical value,which can be used as a sensitive index for the diagnosis of early diabetic nephropathy and an important index for the evaluation of prognosis.
