| Objective The purpose of this study is to explore whether the selection of double lumen tube(DLT)guided by measuring the internal diameter of female cricoid cartilage and left main bronchus(LMB)is better than the selection method of catheter type according to heightafter three-dimensional reconstruction of chest computed tomography(CT).The purpose of this study is to provide reference for the selection of left double-lumen tube(L-DLT)in clinic.Methods 30 patients with right-side thoracoscopic lobectomy were selected in our hospital,aged 35-65 years old,with cardiac function grade I~II and American Standards Association(ASA)grade I~III.According to the traditional height method to determine the trachea model is T group.After CT three-dimensional reconstruction,the type of catheter is selected according to the inner diameter of trachea and bronchus as group C.Compare the changes in heart rate and mean artery pressure(MAP)before and during intubation in each group,and the difference in oxygenation in each group after 5 minutes of one lung ventilation,and record the degree of hoarseness and sore throat at 24 hours after operation.Results There was no significant difference in preoperative general data(age,height,weight)between the two groups(P>0.05).There was no significant difference in preoperative mean blood pressure,heart rate,Sp O2,arterial oxygen and oxygenation index between the two groups.There was no significant difference in the anatomical morphology of cricoid cartilage and LMB between the two groups.There was no correlation between the left and right diameter of cricoid cartilage,the shape of left main bronchus and the height and weight of patients(P>0.05),but there was a positive correlation between the left and right diameter of cricoid cartilage and the equivalent inner diameter of left main bronchus(P<0.05).There was no significant difference in mean pressure,heart rate and preoperative mean pressure and heart rate between the two groups(P>0.05).There was no significant difference in heart rate and mean artery pressure between the two groups(P>0.05).After one-lung ventilation,the OI in both groups decreased in varying degrees,but there was no significant difference between group T and group C(P>0.05).The type of catheter selected in group C was smaller than that in group T,and the difference was statistically significant(P<0.05).There was no significant difference in the air injection volume of bronchial cuff and the accuracy of bronchial part in two groups(P>0.05).The total incidence of hoarseness and sore throat in the two groups was 13.3%and26.7%.There was no significant difference in the incidence of hoarseness between the two groups(P>0.05).The incidence of sore throat in group C was less than that in group T(P<0.05).Conclusion In female patients,the type selection of left double lumen tube guided by chest CT is smaller than that selected by experience,which can reduce the occurrence of postoperative sore throat and provide reference for clinical selection of catheter to some extent. |
