| Objective:To study the effect of surgery on blood glucose in patients with gastric cancer and diabetes and to explore the clinical efficacy of different surgical methods.Methods:Retrospectively collected clinical data of 74 diabetic patients undergoing radical gastric cancer surgery from January 1,2014 to January 1,2019 in the department of the first affiliated hospital of Xiamen university and statistical analysis of relevant clinical data by t test and chi square testResults:Of the 74 patients with gastric cancer and diabetes,one year after the operation.22 cases reached the cure standard,23 cases showed significant improvement,and 29 cases showed no significant improvement.The effective rate was 56.76%.FPG、2hPG、HbA1c were significantly improved in patients at 1 month,3 months,6 months,and 1 year after surgery.The difference was statistically significant(P<0.05).There was no significant correlation between the number of years of diabetes and the effectiveness of the surgery,P>0.05;compared with the preoperative methods for different surgical methods:FPG,2hPG,HbAlc at 3 months,6 months,and 1 year after Billroth Ⅰ patients Significant differences(P<0.05);FPG,2hPG,and HbA1c were significantly different in Billroth Ⅱ patients at 1 month,3 months,6 months,and 1 year after surgery(P<0.05);Roux-en-Y Patients in the anastomosis group had significant differences in FPG,2hPG,and HbA1c at 1 month.3 months.6 months,and 1 year after surgery(P<0.05);3 patients underwent partial gastrectomy in the proximal group There were significant differences in FPG.2hPG,and HbA1c at 1 month,6 months,and 1 year after operation(P<0.05).After 1 year of surgery,the effective rate in the Billroth Ⅰ group was 50%.The effective rate of Billroth Ⅱgroup was 57.69%.The effective rate of Roux-en-Y anastomosis group was 62.96%.The effective rate in the proximal gastric resection group was 45.45%.Conclusion:1.Surgical treatment helps reduce blood sugar in patients with gastric cancer complicated by diabetes,and it begins to take effect in the first month after surgery.2.There is no significant relationship between the number of years of diabetes and the curative effect of surgical treatment of gastric cancer patients with diabetes.3.The four surgical methods,namely,Billroth Ⅰ,Billroth Ⅱ,Roux-en-Y anastomosis,and proximal partial gastrectomy,all improved after surgery compared with those before surgery.4.Through pairwise comparison,the effect of Roux-en-Y anastomosis is most significant.For the surgical treatment of gastric cancer with diabetes,Roux-en-Y anastomosis should be the preferred method of digestive tract reconstruction. |
