Objective:The general situation, clinical features, bilateral ovarian volume, the number of antral follicles, serological indexes, past history of fatty liver disease, smoking and drinking history and ovulation after treatment were compared in patients with polycystic ovary syndrome(PCOS)without abnormal metabolism,PC OS with 1-2 items of abnormal metabolism,PCOS with metabolic syndrome(MS).To observed the effects of metabolic severity on reproductive and endocrine indexes of PCOS patients. To provide the basis for clinical diagnosis and treatment,to improve the quality of life of PCOS patients and to prevent the occurrence of MS.Method:The effect of acupuncture combined with clomiphene on the live birth rate of women with polycystic ovary syndrome(PCOS):according to their metabolic status,1000 patients with PCOS were divided into three groups:PCOS without abnormal metabolism,PCOS with 1-2 items of abnormal metabolism,PCOS with MS.The anthropometric indexes and clinical features of the three groups were recorded:age, weight, body mass index(BMI),waist circumference, hip circumference,waist-to-hip ratio, hairy score, black spinous skin, acne score, menstrual frequency and mean menstrual cycle, serological indicators:sex hormones, sex hormone binding globulin, fasting islets hormone;bilateral ovarian volume and number of antral follicles;past fatty liver disease;smoking and drinking history and ovulation after treatment.SPSS 21.0 statistical software was used for statistical analysis and the conclusion was drawn.Result:1.The results of anthropometric indexes were P<0.05 between the three groups, and the difference was statistically significant. It was indicated that age, weight,BMI,waist circumference, hip circumference, waist hip ratio were positively related to the metabolic status of PCOS patients, the highest levels of PCOS combined with MS,and the lowest of PCOS+0 metabolic abnormalities.2.The comparison between the three groups showed that the black spinous skin was statistically significant(P<0.05).The black spinous skin was positively related to the metabolic status of the polycystic patients, and the heavier the black spinous skin, the heavier the metabolic disorder in the patients with PCOS.The scores of hirsutism and acne, menstrual frequency and mean menstrual cycle showed no significant difference(P>0.05).3.There was no significant difference in the volume of bilateral ovary and the number of antral follicles between the three groups(P>0.05),indicating that the volume of left and right ovaries and the number of antral follicles of left and right ovaries were basically the same among the three groups.4.The results showed that there was a significant difference between the three groups(P<0.05),which indicated that the lower the LH,FSH,LH/FSH,SHBG,the worse the metabolism of PCOS patients. There was a positive correlation between the metabolic status of FINS and PCOS.The results of P,E2,T comparison showed that P>0.05,and there was no statistically difference in the above indexes.5.The comparison of smoking and drinking history among different groups showed that the difference was not statistically significant(P>0.05).6.There was a significant difference in the prevalence of fatty liver among different groups,P<0.05,indicating that fatty liver may increase the risk of metabolic abnormalities in PCOS patients.7.The overall ovulation status after treatment in the three groups and the ovulation status of the clomiphene placebo group showed that the difference was not statistically significant(P>0.05).The difference in ovulation rates between the three groups in the clomiphene treatment group showed a statistically significant difference(P<0.05),indicating that metabolic abnormalities may affect ovulation in PCOS patients.Conclusion:1.Age,body weight, waist circumference, hip circumference and fins were positively correlated with metabolic abnormalities in PCOS patients.2.The higher the metabolic abnormality in PCOS,the lower the hormone level of LH,FSH,LH/FSH,SHBG.3.Fatty liver may increase the risk of metabolic abnormalities in PCOS patients.4.In clomiphene treatment group, the degree of metabolic abnormality was positively correlated with ovulation rate.
