Objective: This study explored the clinical effect and mechanism of BMSCs secreted factor in the treatment of dog ARF,and provided new ideas for the treatment of dog ARF with BMSCs secreted factor.method:Select 32 Chinese rural dogs with roughly the same health status,and after one week of adaptive feeding,they are randomly divided into four groups A,B,C,and D,each with 8 dogs.Group A was used as a blank control;Groups B,C,and D were injected with gentamicin 50 mg / kg to establish an acute renal failure model,Group B was used as a simple model;Group C was used as a furosemide drug treatment group;Group D was used as BMSCs Secreted factor treatment group.Blood routine,biochemical,blood gas,specific gravity of urine,and urine protein content were measured at 1,3,5,7,9,11,13,and 15 days after the start of treatment.B-ultrasound and HE sections of kidney tissue were observed 13 days after treatment.Four groups of dogs after 13 days of treatment were selected,kidney tissue was taken for sectioning,and the distribution and expression of liver regeneration enhancing factor(ALR)and sodium potassium chloride transporter(NKCC)were observed by immunohistochemistry.Test results:The medical treatment group(group C)and BMSCs secretion factor treatment group(group D)were compared.Group C recovered after 15 days of treatment,and group D cured around 11 days.A The mortality rate was 0% in group B,100% in group B,50% in group C,and 37.5% in group D;the biochemical indicators of blood creatinine and urea nitrogen both showed a downward trend,and the blood creatinine in group C was 5th.The day began to decline,and the difference between group B and group B was significant on the 7th day(p <0.05),and the difference between group A and group B on day 11 was extremely significant(p <0.01).After 7 days of treatment,the difference with group B was extremely significant(p <0.01),and after 11 days of treatment,it fell to the normal range,and the difference with group A was not significant(P> 0.05).The urea nitrogen of group C began to decrease on day 5,The rate of decline is slow,the difference between Group A and Group A is very significant D,and clear boundary of cortical medulla;HE tissue staining section observation showed that the glomeruli and balloon structures of the kidneys in group A were normal,the tubular epithelial cells were normal,and the morphology of the proximal and distal tubules was normal;group B,The glomerular atrophy is severe,the blood vessels adhering to the balloon are severely bleeding,a large number of inflammatory cells,the epithelial cells are disordered and protein tubes appear;Group C can be observed with blood vessels bleeding,the cortical medulla junction is not clear,and vacuolar degeneration;There is less atrophy of the glomerulus,a small amount of blood vessel bleeding,and individual protein casts.The positive reaction of ALR in group A is not obvious,all the tissues of glomerular cortical medulla are normal,and no positive reaction can be seen in all parts.The IOD value is low.The positive reaction of glomeruli and dense plaques in group B is obvious,and the cortical pulp The qualitative part is obviously yellow,the positive reaction is strong,the renal tubular epithelial cells are shed and have a positive reaction,the IOD value is the highest and the difference is very significant from group A(p <0.01);the positive reaction of glomeruli in group C is more obvious,but weaker than In group B without epithelial cell shedding,some positive reactions in the distal tubules were less intense than in group B.The IOD value was higher and significantly different from group A(p <0.05),and significantly lower than group B(p <0.05)In group D,the positive reaction of glomeruli,cortex and medulla was not obvious,and the tissue structure was normal.There was a slight positive reaction compared to group A,but it was not obvious compared to groups B and C.The IOD value was close to group A and was significantly different from group B(p <0.01).NKCC2 showed no positive reaction in glomeruli,distant tubules,and proximal tubules in group A.The IOD value was low.The glomeruli in group B were more positive than those in group A,but the most obvious manifestations were the distal tubules and medulla.Part of the myeloid loops and collecting duct epithelial cells are yellow and have a strong positive reaction.The highest IOD value is significantly different from that in group A(p <0.01).The renal tubules in group C have a positive reaction,but the area is not large.The expression was enhanced,and the IOD value was significantly different from that in groups A and B(p <0.05).Group D was the closest to group A.The positive reaction in the renal tubules was mild.Compared with group C,the positive reaction was not obvious,and there were no positive cells in the cortex.The IOD value was not different from group A(P> 0.05),and was significantly different from group B(p <0.01).Conclusion:1.The ARF effect of the BMSCs secretion factor treatment group was better than that of the furosemide conventional drug treatment group.2.The effective treatment of ARF by secreted factors of BMSCs is related to the inhibition of the expression of ALR and NKCC.
