Vascular interventional embolization is widely used in the treatment of arteriovenous malformations,arteriovenous fistulas,aneurysms and other vascular diseases as a single palliative treatment or preoperative adjuvant treatment because of its advantages of wide application,small injury,quick recovery,good curative effect and small side effects.Cyanoacrylate is one of the most commonly used liquid embolic agents in clinical.It has the advantages of strong adhesion,fast polymerization rate,good fluidity and strong permeability.Therefore,it is often used in the treatment of intracranial or other parts of bleeding,tissue ischemia,arteriovenous malformation and high fluidity vascular disease embolization.In the process of interventional operation,the embolic glue is often mixed with iodine oil.The mixing concentration and the kinetics of polymerization reaction with blood play an important role in the embolization effect and safety of operation.However,the proportion of embolic gum and iodine oil depends on the experience and operation habits of clinicians.The quantitative relationship between the proportion of embolic gum and iodine oil,blood composition and polymerization rate is not clear.The existing methods can only describe the polymerization process of the film,and it is not enough to simulate the in-situ polymerization process of the embolic adhesive.Therefore,the existing methods based on Raman spectrum and high-speed photography are improved,The content and results of the kinetic process of polymerization of the mixture of embolization gum and iodine oil were studied:(1)A method for characterizing the kinetics of embolic polymerization was established based on Raman spectroscopy and high speed photography.In the study of Raman spectroscopy,the Raman spectrum of the sample points of polymerization reaction in glass tube is monitored by Raman spectrometer,and the peak height of the spectrum is obtained by data processing.In the research based on high speed photography,the polymerization process of embolic adhesive was characterized by recording the change of light transmittance of the adhesive sample.(2)Based on the research method designed in this paper,the polymerization kinetic process of embolic glue was detected.In this part,the effects of volume ratio of rubber to oil and composition of blood simulant on the polymerization kinetics of embolic glue were considered.The results showed that the higher the concentration of lipiodol in the mixture,the stronger the inhibition on the polymerization;The higher the concentration of protein in the blood simulant,the more significant the promoting effect on the polymerization of embolic glue.In conclusion,the volume ratio of rubber to oil and the composition of blood simulant all have influence on the polymerization kinetics of embolic glue.(3)The results of Raman spectroscopy and high speed photography are compared.Firstly,the experimental data obtained by the two optical methods are compared and verified.The results show that although there are differences in the change of the two methods,there is no significant difference in the total polymerization time of the embolic adhesive obtained by calculation.Then,the limitations of the two methods in the kinetics of the polymerization of the embolic adhesive were analyzed.The results show that Raman spectroscopy can characterize the polymerization of embolic gum in essence,but the sampling period of this method is long and affected by noise interference;Although the high-speed photography can improve the sampling period,it can not represent the polymerization of embolic adhesive in essence.In conclusion,two experimental devices based on different optical methods were designed,which can measure the polymerization kinetics of embolic glue under different experimental conditions.Through the comparative study of the two methods,we can explain the limitations and advantages of the two methods.As a non-invasive detection method,optical method has been widely used in the medical field.The characterization of the polymerization kinetics of embolic glue in this study can provide important reference for clinicians and is of great significance for the safe and effective treatment of blood vessels. |
