Objective: Mechanical stimulation mainly includes tensile stress stimulation and compressive stress stimulation.suitable tensile stress stimulation has been used in many fields,which can promote fracture repair,distraction osteogenesis and orthodontic tooth movement.many studies have confirmed the effect of appropriate tensile stress stimulation on osteoblasts.However,there is no high-level evidence-based medicine evidence to prove the effect of compressive stress on osteoblasts and osteoclasts.Therefore,this study collected the relevant literature on the effects of compressive stress stimulation on the growth of osteoblasts and osteoclasts,and integrated the data.The aim of this study is to systematically evaluate the effects of compressive stress on the growth of osteoblasts and osteoclasts.Methods: Pub Med,Embase,Cohrane Library,China knowledge Network,China Biomedical Literature Database,VIP and Wan Fang Database were searched by computer.The search language is limited to Chinese and English,and the effects of compressive stress stimulation on the growth of osteoblasts and osteoclasts were included.The alkaline phosphatase content,cell survival rate,apoptosis rate,cell count,cell proliferation ability and tartrate-resistant acid phosphatase content of osteocytes after compressive stress stimulation were taken as the outcome indexes of the study.the literature was screened strictly according to the inclusion and exclusion criteria,and the literature with low quality or no research outcome index was excluded,and the final included literature was extracted and evaluated.The data extracted in this study were analyzed by Meta using stata16.0 software.Results: A total of 22 related articles were included in this study,including 17 articles about osteoblasts and 5 articles about osteoclasts.The results of Meta analysis showed that the content of alkaline phosphatase expressed by osteoblasts stimulated by compressive stress was significantly higher than that in the control group [SMD=3.29,95%CI(2.34,4.24),P<0.05].There was no significant difference in cell survival between the compressive stress stimulated osteoblasts group and the control group [WMD=1.94,95%CI(-7.34,11.22),P>0.05].There was no significant difference in apoptosis rate between the osteoblast group and the control group [WMD=1.39,95%CI(-3.23,6.01),P>0.05].There was no significant difference in cell count between the compressive stress stimulated osteoblasts group and the control group [SMD=175.79,95%CI(-154.36,505.94),P>0.05].The proliferation ability of osteoblasts stimulated by compressive stress was higher than that of the control group,and the difference was statistically significant [WMD=1.64,95%CI(0.84,2.44),P<0.05].The expression of tartrate-resistant acid phosphatase in osteoclasts stimulated by compressive stress was lower than that in the control group,and the difference was statistically significant[WMD=-2.16,95%CI(-2.83,-1.49),P<0.05].There was no significant difference in cell count between the compressive stress stimulated osteoclast group and the control group [WMD= 5.12,95%CI(-3.66,13.89),P>0.05]Conclusion: According to the existing literature evidence,it can be found that appropriate compressive stress stimulation can improve the proliferation ability of osteoblasts,promote the repair of bone injury,inhibit the activity of osteoclasts and reduce bone resorption.
