In recent years,as the rehabilitation medical career in China is designed on top national level and pushed forward rapidly,a complete rehabilitation medical system has been formed and it has played an important role in the entire lifecycle,which helps build a medical organization structure with trinity of prevention,clinic and rehabilitation.With the people grow their demand for rehabilitation,not only the development of assistive devices on rehabilitation is getting faster and faster,but also the demand for them is growing rapidly.However,since the assistive devices on rehabilitation are on a starting stage in China,people have less understanding on the device and its adaptive service,and the safeguard policies from government are still not sound,so the adaptive rate of rehabilitation assistive devices is relatively low.Based on this background,this study takes a big size public-owned rehabilitation hospital in Shanghai as subject to research on the service demand from patients for rehabilitation assistive devices and their quality,which will stand on economic point of view of cost-effectiveness analysis to better explore and discover whether the rehabilitation assistive devices follow the principle of cost-effectiveness on rehabilitation,aiming to improve the adaptive rate and effectiveness in rehabilitation hospital.According to the theory and evaluation method of medical service quality,the research uses the methods of literature analysis,questionnaire survey,comparison analysis,KANO model and cost-effectiveness analysis to explore the potential demand from the rehabilitation patients for adaptation service on rehabilitation assistive devices,and to evaluate the two economic schemes of adapting rehabilitation assistive devices and not adapting rehabilitation assistive devices.The results of research show: 1.Cost-Effectiveness Analysis: after more than one month training,the experimental group that adapt to rehabilitation assistive devices have higher difference value between MBI index and Berg index than the control group that not adapt to rehabilitation assistive devices and they have statistic difference.The total cost for experimental group is lower than that for control group.The average cost for experimental group is 61701.27 yuan,while the average cost for control group is 88510.20 yuan,which have statistic difference.Every increase of 1 score of MBI and Berg for experimental group requirelower cost than control group,which have statistic difference.2.The Analysis of KANO Model on Service Demand: the survey results show that there are 11 expectation demands,8 charm demands and 1 undifferentiated demand in all the factors on service quality,in which the expectation demand accounts for 55% of the total,the charm demand accounts for 40% and the undifferentiated demand accounts for 5%.Satisfaction Impact and Dissatisfaction Impact Results are: The top three SI values are rehabilitation effect,service attitude and encouragement,while the top three DSI values are adjustment plan,service attitude and cost explanation.There are 3 conclusions through the KANO model and Cost-Effectiveness Analysis,as follows:1.Adaptation Cost for Rehabilitation Assistive Devices-Evident Performance The cost-effectiveness ratio of MBI and Berg for the experimental group is lower than that for the control group,which shows that,the patients from the experimental group paid extra costs for making and adapting rehabilitation assistive devices.The rehabilitation performance is raised significantly and the hospitalization period is also reduced.2.Rehabilitation Focus on Attitudes of Service Among the statistical results for KANO questionnaire,attention shall be paid firstly to three quality attributes: essential attributes,charm attributes and expectation attributes.This survey shows that,there are no essential attributes in 20 services,while the expectation attributes account for 55%,which shows that the services in this attribute are the potential demands from rehabilitation patients,and they are looking forward to have more than half of the rehabilitation assistive devices with adapting services.3.Rehabilitation Patients Shows A Younger Trend The questionnaire survey shows that,in the 250 interviewers,there are 101 people with ages ranging from 31-50 years old,which is the highest,accounting for 43.91% of the total,followed by 66 patients aging 51-65,accounting for 28.70%.
