Abstract/Summary:
Objective:To verify the non-inferiority of snap needles compared with filiform needles in the treatment of migraine attacks, and to provide preconditions for exploring the accessibility of snap needles. Methods:The cases meeting the inclusion criteria were randomly divided into 30 cases in the test group(pinch needle group)and 30 cases in the control group(fine needle group).Press acupuncture needles to take bilateral acupoints, embed the needles for 48 hours, press once for the first half an hour and ten minutes, then do not press, take acupuncture points from both sides with a needle, leave the needle for half an hour, and perform acupuncture every ten minutes.Record the onset time, treatment start time,30 minutes,1 hour,2 hours,4 hours, and 24 hours of headache visual analog score, pain degree classification, dysfunction, and accompanying symptoms.Record adverse events at any time. The SPSS23.0 statistical analysis software was used to perform statistical analysis on the general condition, baseline and efficacy of the two groups of cases. The main curative effect index is the rate of halving the degree of headache in 2 hours, and the secondary index is the ratio of patients with disappeared pain, recurrence rate, headache degree grade, dysfunction score, and accompanying symptom score. And analyze the patient’s overall evaluation of treatment and the investigation of the accessibility of treatment methods. Results:A total of 53 patients completed the entire study cycle, including 25 in the experimental group and 28 in the control group.(1)General situation:The two groups of patients were comparable in age, course of disease,VAS score 4 weeks before treatment,VAS score at onset, headache level, dysfunction score, and accompanying symptom score,P>0.05.(2)Main efficacy indicators:Compared with the two-hour headache halving rate between the two groups,P>0.05,no significant difference, and comparable. The 2-hour headache halving rate(52%)of the press-acupuncture group was slightly lower than the 2-hour headache halving rate of the fili-needle group(53.6%).(2)Secondary curative effect indicators:①Comparison of the ratio of the number of headache disappearances between the two groups,P>0.05,there was no significant difference, and it was comparable. The ratio of headache disappearance patients in the press acupuncture group(28%)was lower than that in the milli-needle group(39.29%).②Comparing the recurrence rate of headache between the two groups,P>0.05,there is no significant difference, and it is comparable. The needle group The recurrence rate(0)was lower than the recurrence rate of the filiform needle group(7.14%).③Compared with the grades of headache degree between the two groups,P>0.05 was comparable at 4.0h and 48h after treatment. The improvement in the grades of headache degree was slightly lower than that of the milli-needle group. At the 0.5h,1.0h,and 2.0h observation time points after treatment,P<0.05,there is a significant difference.④Dysfunction score:The scores of the two groups before treatment are significantly different. The dysfunction scores of the past 4 weeks are missing. The results obtained at each observation point after treatment cannot fully explain the difference in efficacy between the two.⑤ Concomitant symptom score:0.5h after treatment, the improvement of concomitant symptoms in the press acupuncture group is not as good as that of the fili-acupuncture,and the improvement in the concomitant symptoms score of the press-acupuncture group at other points is not much different from that of the fili-needle group.(4)Patient evaluation index:①Efficacy evaluation:the patients’ evaluation of the two groups’efficacy is comparable, and the patients’ think that the method they each anttend is good.②Availability survey:The two groups of patients believed that the accessibility of punched needles was stronger than that of filiform needles.in conclusion:(1)Comparing the halving rate of 2h visual analogue score, the effect of pressing needle and filiform needle is not much different.(3)In the control of recurrence rate, the press needle group is better than the mill needle group.(4)Press has a certain effect on improving patients’accompanying symptoms.(5)The safety of the press needle is good, and no adverse reactions have been found. Therefore, for patients with migraine attacks, those with mild to moderate pain can use snap needles as an alternative treatment for filiform needles.
