BackgroundPolycystic ovary syndrome(PCOS)is one of the most prevalent endocrine disorders in gynecology.PCOS may lead to various reproductive endocrine disorders and long-term health complications,and is therefore a potential threat to the health of women.In recent years,gut microbiota has been the research hotspot,which have an essential role in human metabolic regulation.Researches show that gut microbiota diversity decreases in PCOS,which can make lipopolysaccharide into the circulatory system,Promote phosphorylation of insulin receptor substrate 1 in insulin signaling pathway,then promote inflammation,lead to insulin resistance.BuShenHuaZhuo Prescription(BSHZ)could treat PCOS,but the mechanism was not clear?ObjectiveTo investigate the mechanism of BSHZ combined with fecal microbiota transplantation(FMT)in polycystic ovary syndrome(PCOS)induced by letrozole in rats by weight,ovary tissue,testosterone(T),luteinizing hormone(LH)/follicle stimulating hormone(FSH),Fasting insulin(FINS),Fasting plasma glucose(FPG),HOMA-IR,Interluekin(IL)-18,Tumor Necrosis Factor(TNF)-? and gut microbiota?Method1.Grouping:Thirty six-week old SD rats were divided into control group,model group,traditional Chinese medicine(TCM)group,FMT group and TCM combined with microbiota transplantation(TCM+FMT)group,with 6 rats in each group.2.Testing:After the treatment,abdominal aortic blood was taken to detect the levels of T,TL/FSH.FINS,FPG,HOMA-IR.The levels of serum IL-18 and TNF-? were detected by ELISA.The ovary tissue was observed by HE staining.The diversity of gut microbiota was detected by 16S rRNA technique.Result1.Body weight level,compared with control group,the weight of the rats increased significantly in model group(P<0.01).Compared with model group,the weight of rats in TCM and TCM+FMT group decreased(P<0.05),but there was no significant difference in FMT group(P>0.05)2.Ovary tissue level,compared with control group,there were more cyst-like follicles in ovary tissue in model group.The number of small cyst-like follicles was significantly decreased,and the corpus luteum or large follicles were observed after ovulation in TCM,FMT and TCM+FMT groups,.3.Sex hormone level,compared with control group,T and LH/FSH increased significantly in model group(P<0.01).Compared with model group,T and LH/FSH decreased significantly in TCM,TCM+FMT group(P<0.01),while T and LH/FSH decreased in FMT group(P<0.05).4.Metabolic level,compared with control group,FPG and HOMA-IR increased significantly in model group(P<0.01),while FINS had no significant difference(P>0.05).Compared with model group,FPG and HOMA-IR decreased significantly in TCM group(P<0.01),FPG and HOMA-IR decreased significantly in TCM+FMT group(P<0.05),FPG and HOMA-IR had no significant difference in FMT group(P>0.05),FINS had no significant difference in TCM,FMT and TCM+FMT group(P>0.05).5.Inflammatory factor level,compared with control group,IL-18 and TNF-aincreased significantly in model group(P<0.01).Compared with model group,IL-18 decreased significantly in FMT,TCM+FMT group(P<0.001),IL-18 had no significant difference in TCM group.TNF-? decreased significantly in TCM+FMT group(P<0.01),and there was no significant difference in TCM and FMT group(P>0.05).6.Gut microbiota level,compared with control group,the abundances of lactoacillus,bacteroidales,prevotellaceae and muribaculum on Genus were lower in model group,the abundances of ruminococcaceae,lachnospiraceae and clostridiales on Genus were higher in model group(P<0.05).Compared with model group,the abundances of lachnospiraceae and ruminococcaceae on Genus were lower in model group,the abundances of muribaculum and prevotellaceae on Genus were higher in TCM group,FMT group and TCM+FMT group(P<0.05).the abundances of lactoacillus on Genus was higher in FMT group,the abundances of bacteroidales on Genus was higher in TCM group(P<0.01).Conclusion1.TCM combined with fecal microbiota transplantation can significantly improve the reproductive function in the number of cyst-like follicles,T,LH/FSH of PCOS rats.2.TCM combined with fecal microbiota transplantation can significantly improve the metabolism in weight,FPG?HOMA-IR of PCOS rats3.TCM combined with fecal microbiota transplantation can significantly improve the chronic inflammatory state in IL-18,TNF-? of PCOS rats.4.PCOS rat model has the phenomenon of dysfunction of gut microbiota,which means that the PCOS rat model has lower abundances of beneficial bacteria including lactoacillus,bacteroidales,prevotellaceae and muribaculum,and the higher abundances of deleterious bacteria including lachnospiraceae and clostridiales.5.TCM and FMT can improve the phenomenon of dysfunction of gut microbiota,which means can increase abundances of beneficial bacteria including muribaculum and prevotellaceae,decrease abundances of beneficial bacteria including lachnospiraceae of PCOS rats.
